Non‑Medicare members — Indications where CT colonography is indicated

CT colonography is indicated only when the patient has ONE of the following qualifying conditions:

Qualifying indications: 1) Coagulation disorder known to increase bleeding risk; 2) Instrument (optical) colonoscopy of the entire colon is incomplete and/or colonoscopy is contraindicated due to colon obstruction; 3) Lifetime or long-term anticoagulation with increased patient risk if therapy is discontinued; 4) Significant medical or surgical complications from previous standard (optical) colonoscopy; 5) Medical condition that places the patient at increased risk with use of conscious sedation

Medicare members — Screening after positive FIT/FOBT

For Kaiser Permanente Medicare Advantage members, CT colonography for colorectal cancer screening after a positive FIT/FOBT may be considered medically necessary only when the patient also meets the non‑Medicare qualifying criteria above.

Historically, use of virtual colonoscopy as a general colorectal cancer screening tool did not meet Kaiser Permanente’s Medical Technology Assessment Committee (MTAC) criteria. MTAC reviews (2001–2004) concluded that virtual colonoscopy was less effective than conventional colonoscopy for detecting colorectal polyps and carcinomas overall, was more reliable for larger lesions (>10 mm) but not proven equivalent for population screening, and therefore failed MTAC diagnostic test criteria for routine screening use.
