Medically Necessary Criteria for Home INR Monitoring

Covered when ALL of the following are met:

Eligibility conditions: Patient has mechanical heart valve(s) OR chronic atrial fibrillation OR venous thromboembolism and meets Medicare coverage criteria.

Service requirements: Prior to initiation, a face-to-face demonstration of monitor use is performed, at least one blood sample is obtained, instruction is provided for reporting home INR results, and the patient's ability to perform testing and report results is documented.
