Medically Necessary Criteria

Medically Necessary Coverage Criteria for Dental Anesthesia (Self-Funded and specified plans)

Facility and anesthesia charges may be covered if the member meets established, documented criteria including, but not limited to one or more of the following:

Patients for whom local anesthesia is ineffective because of acute infection, anatomic variations, or allergy
Patients who cannot cooperate due to a lack of psychological or emotional maturity and/or mental, physical, or medical disability
Patients who are extremely uncooperative, fearful, unmanageable, anxious, or uncommunicative with dental needs of such magnitude that treatment should not be postponed or deferred and for whom lack of treatment can be expected to result in dental or oral pain, infection, loss of teeth, or other increased oral or dental morbidity
Patients for whom the use of general anesthesia may protect the developing psyche and/or reduce medical risk
Patients requiring significant surgical procedures
Patients requiring immediate, comprehensive oral/dental care, such as trauma related care
Pediatric patients (requests will be reviewed by the Office of the Medical Director)

Contraindications / Not Covered

General anesthesia for dental services is contraindicated and NOT COVERED for any of the following:

A patient with predisposing medical conditions which would make general anesthesia inadvisable
Healthy, cooperative patient with minimal dental needs

Claims for general anesthesia for dental services may be denied when general anesthesia is contraindicated — specifically when the patient has predisposing medical conditions that make general anesthesia inadvisable — or when the patient is a healthy, cooperative individual with minimal dental needs.

X-rays provided in a hospital or ambulatory surgery suite

Inpatient payment may be made when the individual, because of an underlying medical condition or clinical status or because of the severity of the dental procedure, requires hospitalization in connection with provision of dental services.

When outpatient office treatment is unsuccessful or inappropriate due to behavioral or medical complexity, medical record documentation is required for coverage review. Documentation should include the treatment plan, the patient's health history, the date(s) treatment was attempted, and the patient's response.

Pre-authorization and review practice

Preauthorization is not required at present for these services; the medical necessity criteria will be used upon retro-review. Preauthorization is required for Self-Funded members. Pediatric requests will be reviewed by the Office of the Medical Director.

Background: Coverage is limited to situations where, after an unsuccessful attempt or when inappropriate to treat in the dental office because of the severity of a behavioral issue, hazardous medical condition, or treatment complexity, inpatient or ambulatory setting care and anesthesia may be medically necessary. Documentation must include the treatment plan, health history, dates treatment was attempted, and the patient's response. The policy bases its medical necessity criteria on a review of peer-reviewed scientific literature and applicable state and federal guidance.
