Medical Necessity Criteria for MAC

Monitored anesthesia care may be considered medically necessary when ONE or more of the following higher risk situations are documented by the anesthesiologist:

ANY of the following

Increased risk for complications due to severe co-morbidity (ASA class 3 physical status or greater)ASA >= 3
Severe sleep apnea that requires use of a positive pressure airway device during sleep
Morbid obesity (Body mass index >40)BMI > 40
Increased risk of airway obstruction due to anatomic variation (e.g., abnormalities of the oral cavity, neck or jaw, dysmorphic facial features)
Spasticity or movement disorder complicating procedure
Inability to follow simple commands due to cognitive dysfunction or psychological impairment
History or anticipated intolerance to standard sedatives due to chronic opioid or benzodiazepine use
Active medical problems related to drug or alcohol abuse
Age older than 70> 70 years
Age younger than 18 years< 18 years
Pregnancy
Prolonged or therapeutic procedure requiring deep sedation
Documented history of previous problems with anesthesia or sedatives (e.g., failure of moderate sedation)
Acutely agitated or uncooperative patients (e.g., delirium, organic brain disease, dementia)

Anesthesiologist must document in the member’s medical record that one or more of the listed higher-risk situations exist to support use of monitored anesthesia care (MAC). Claims will be denied if clinical documentation does not indicate compliance with the clinical criteria.

Anesthesia claims are subject to post-service retrospective (pre-pay) review and will be denied on retro-review if the provided clinical documentation does not demonstrate compliance with the policy’s medical necessity criteria.

Background and Evidence

Background: Monitored Anesthesia Care (MAC) is an anesthesia service provided by a qualified anesthesia clinician that includes a pre-procedure assessment/optimization, intraprocedural monitoring and support of vital functions, diagnosis and treatment of clinical problems that occur, administration of sedatives/analgesics as needed, post-procedural management, and conversion to general anesthesia if required. MAC permits safe administration of deeper levels of sedation than proceduralist-directed moderate sedation.

Rationale: National guidelines (for example, American Society of Anesthesiologists and ASGE guidance, systematic reviews and UpToDate summaries) support the use of MAC for outpatient endoscopic procedures in patients with certain risk factors or significant medical conditions. Independent Health therefore limits MAC to situations where an anesthesiologist documents one or more specified higher-risk indications to meet medical necessity.

Evidence and guideline references: Guideline and evidence sources cited include ASA position statements distinguishing MAC from moderate sedation, ASGE guidelines, a systematic review by Adams and Saleh, UpToDate, and relevant CMS and state Medicaid references.

Definitions: Monitored Anesthesia Care (MAC) — an anesthesia service provided by a qualified anesthesia provider that includes pre-procedure assessment/optimization, intraprocedural care and postprocedural management, diagnosis and treatment of clinical problems that occur during the procedure, support of vital functions (including airway and hemodynamic management), and administration of sedatives/analgesics/anesthetic agents as necessary. Moderate sedation — a proceduralist-directed service that does not include a qualified anesthesia provider's periprocedural assessment; the proceduralist typically performs the procedure and supervises sedation. ASA physical status classification — ASA 1: normal healthy patient; ASA 2: mild systemic disease; ASA 3: severe systemic disease; ASA 4: severe systemic disease that is a constant threat to life; ASA 5: moribund patient not expected to survive without operation; ASA 6: brain-dead donor.
