Reimbursable serologic testing criteria
Two-tier serologic testing is reimbursable when performed for specified clinical presentations consistent with possible Lyme disease.

ANY of the following
- Acute cranial neuropathy
- Acute myocarditis, pericarditis or cardiomyopathy
- Ataxia
- Cough or shortness of breath
