Initial CPAP coverage documentation
Covered when ALL of the following are confirmed on the Certificate of Medical Necessity (CMN):
Form requests entry of Apnea‑Hypopnea Index (AHI) or Respiratory Disturbance Index (RDI).
Form includes tri-state response for correction of dysrhythmia(s) (Y/N/D).
