Providers may request a formulary exception for a prescribed nonformulary contraceptive drug for medical necessity via the online request form. When the exception is approved the drug is provided with a zero-dollar cost share. An initial 'fail-first' is not required; provider-determined medical necessity is sufficient.

Only FDA‑approved contraception apps available for cell phone download are reimbursable through the paper claim process with a prescription. Members must submit three documents to obtain reimbursement and the claim should note 'contraception app purchase' in the diagnosis section.
