Documentation criteria for episodes of care

Documentation for episodes of care must meet the following minimum standards unless superseded by provider, state, federal, or CMS contracts and/or requirements.

Documentation must be legible to someone other than the writer.

Documentation must be complete, dated, and timed; entries must include author identification (handwritten signature, unique electronic identifier, or initials and credentials as applicable).

Documentation must reflect all aspects of care; information identifying the member must be included on each page of the medical record.

Timely entry of information into the medical record is expected to be completed at the time of service or shortly thereafter and should not exceed 30 days; signature date must be within 30 days of the date of service; include signature time for services performed in a hospital setting.

Operational expectation: signature date within 30 days and signature time for hospital services.

Denial Risk

Ensure authorizations, medical necessity, and coding are supported in the record

Providers must follow authorization and medical necessity guidelines and submit industry-standard CPT/HCPCS/revenue codes that are fully supported in the medical record. Claims that are not supported by documentation or that do not follow coding/billing guidelines may be rejected, denied, adjusted, or subject to recovery/recoupment.

Use CPT, HCPCS, and/or revenue codes as appropriate and ensure coding is fully supported in the medical record.
If appropriate coding/billing guidelines or current reimbursement policies are not followed, Highmark BCBS may reject or deny the claim, recover/recoup payment, or adjust reimbursement to reflect appropriate services.
