Diagnosis codes (ICD) are specific and appropriate; avoid non‑specific 3‑digit ICD codes that may be denied for insufficient specificity.
Service date and place of service are included with appropriate Place of Service (POS) codes.
Claims missing required information may be denied.

ALL of the following

If a code falls under an NCCI PTP edit, it must be billed in alignment with the edit; PTP edits prohibit billing mutually exclusive procedures together.
Modifiers may only be used when appropriate for PTP edits; Modifier 59 may be used only if no other appropriate modifier describes the service.
If a code falls under an MUE, billed units must not exceed the maximum units allowed per the NCCI Manual.

ALL of the following

If an approved PLA or MAAA code exists for a proprietary or multianalyte test, that PLA/MAAA code must be used and may be billed only for the specific test assigned to that code.
If a panel code applies, the panel code must be used and it is inappropriate to bill separately for subcomponents of that panel without an appropriate modifier or exception.
When a consolidated procedure (e.g., NGS or tandem mass spectrometry) evaluates multiple genes/analytes, report one unit of the appropriate sequencing/panel code per NCCI guidance; Modifier 52 may be used to indicate reduced wet lab procedures where applicable.

For CPT ranges 81105-81479, 81490-81599 and applicable PLA codes, include the Concert GTU in the PA submission; if a GTU was obtained, include it on the prior authorization request.
