General Inflammation Testing (CRP and ESR) Coverage Criteria
This policy governs coverage criteria for measurement of C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) for inflammatory conditions and related indications for Healthfirst members in North Carolina. It specifies when testing meets coverage criteria, test preferences, and frequency recommendations for listed conditions.
Removed the word "inflammatory" from CC1 and clarified that measurement of CRP and/or ESR meets coverage criteria for the conditions specified in Note 1.
If CRP and ESR are ordered at the same time for a condition where CRP or ESR are allowed, only CRP will be approved.
In the Note 1 table, added an example parenthetical "(e.g., inflammatory bowel disease)" in the Frequency of Testing column for IBS for clarity.
