Coverage Criteria for CRP and ESR

Multisystem Inflammatory Syndrome in Children (MIS-C) workup

Covered when ALL of the following are met:

Patient is a child or adolescent with suspected Multisystem Inflammatory Syndrome in Children (MIS-C) following SARS-CoV-2 infection or exposure
Initial (tier 1) laboratory evaluation includes: complete blood count with manual differential, comprehensive metabolic panel, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and SARS-CoV-2 testing by PCR or serology
Tier 1 results demonstrate either CRP ≥ 25 mg/L or ESR ≥ 40 mm/hr AND at least one additional suggestive laboratory abnormality (e.g., neutrophilia, lymphopenia, thrombocytopenia, hyponatremia, hypoalbuminemia)
If tier 1 criteria above are met, patient proceeds to tier 2 evaluation which includes electrocardiogram (EKG) and echocardiogram
Testing is used as part of the diagnostic workup in conjunction with clinical assessment and is not used in isolation to confirm or exclude MIS-C

Billing Rule

Ordering CRP and ESR together

Ordering both CRP and ESR simultaneously is generally discouraged when one marker is sufficient; when either test is allowed, CRP is preferred. If both are ordered for a condition where either is acceptable, only CRP will be approved and some outpatient labs may perform only CRP unless a written indication for ESR is provided.

When policy allows either CRP or ESR, order CRP as the preferred first-line test.
If both tests are submitted without an indication that requires both, reimbursement may be limited to CRP.
Outpatient labs may require a written indication on the requisition to perform ESR.

Prefer CRP for acute inflammation

Prefer CRP over ESR for evaluation of acute-phase inflammation and many infections due to faster rise and fall and slightly superior diagnostic accuracy in some studies.

CRP elevates within 24 hours of inflammatory stimulus and normalizes more rapidly than ESR.
When CRP and ESR offer similar utility for chronic conditions, follow guideline-specific recommendations.
