Reimbursement Criteria for Unlisted Procedure Codes

Unlisted procedure code reimbursement criteria

Claims with unlisted procedure codes will be processed according to availability of a more specific code, adequacy of supporting documentation, and other billing rules.

Do not use an unlisted procedure code when a more descriptive specific code exists; select the procedure name that accurately identifies the service performed per CPT instructions.

If no appropriate specific code exists, report the service using the appropriate unlisted CPT/HCPCS code and include any applicable modifiers or extenuating circumstances.

Claims must include supporting documentation that clearly describes the nature, extent, and medical necessity of the procedure or service; designate the portion of the report that identifies the test or procedure associated with the unlisted code.

Multiple same unlisted CPT uses — reporting guidance

RuleWhen performing two or more procedures that require the use of the same unlisted CPT code, report that identical unlisted CPT code only once to identify the services provided (this exclusion does not apply to unlisted HCPCS codes).

ExclusionThis single-report rule excludes unlisted HCPCS codes (for example, DME or unlisted drugs).

RationaleReporting the identical unlisted CPT multiple times is not required and may lead to review; the policy instructs a single code to identify multiple same unlisted CPT procedures.

Authorization is required when an unlisted procedure is considered experimental. Submit prior authorization per payer guidelines before performing experimental procedures to avoid denial.

Denial Risks for Unlisted Procedure Claims

Claims submitted with unlisted procedure codes without supporting documentation are denied. Claims are also denied if, after review, a more appropriate procedure code is available. Unlisted procedure codes with modifiers appended are reviewed and may be denied. Additional reimbursement is not provided for techniques or equipment submitted with an unlisted procedure code. When multiple procedures use the same unlisted CPT code, report the unlisted code only once (this excludes unlisted HCPCS codes). If services are considered bundled, the provider will be notified and additional payment denied.

When submitting a claim that includes unlisted procedure codes, provide supporting documentation that clearly identifies and justifies the service. Required documentation must designate the portion of the report that identifies the test or procedure associated with the unlisted code. Include the following as applicable: a detailed description of the nature, extent, and medical necessity of the procedure or service; whether the procedure was performed independently or at the same surgical site/through the same surgical opening as another procedure; an account of time, effort, and equipment required; description of any extenuating circumstances that complicated the service; and the number of times the procedure was performed or services provided.
