Add-on code adjudication

Reimbursement and adjudication criteria for add-on codes:

Add-on codes must be billed in conjunction with an appropriate primary procedure code on the same claim or be present in claims history for the same date of service and the same provider.

The Health Plan follows AMA and CMS guidance regarding reporting and adjudication of add-on CPT and HCPCS codes.

Code editing evaluates the current claim and claims history for presence of the associated primary code; if the primary code is not present on the current claim or in history, the add-on service line is denied by automated edit.

If the primary procedure code is present but has been denied for another reason, the associated add-on code will also be denied.

Do not bill add‑on CPT/HCPCS codes as standalone services. Code editing software will deny an add‑on service line if the associated primary procedure code is not present on the current claim or in claims history for the same date of service and provider; if the primary code is present but denied, the add‑on will also be denied.

Add‑on codes must be billed only in conjunction with an appropriate primary procedure on the same claim or be present in claims history for the same DOS and provider.
If the primary procedure is denied for any reason, the associated add‑on code will be denied by edits.
