Initial - ED: 1) Diagnosis of erectile dysfunction; 2) Age ≥ 18 years; 3) If brand Viagra or Vybrique is requested, member must use generic sildenafil (25 mg, 50 mg, 100 mg) unless contraindicated or clinically significant adverse effects are experienced; 4) Sildenafil (Viagra, Vybrique) is NOT prescribed concurrently with nitrates or guanylate cyclase stimulators; 5) Dose does not exceed both: a) 100 mg per day; b) Health plan approved quantity limit; Note: Therapeutic failure does not constitute acceptable medical justification. Illinois HIM exception for Vybrique step therapy as of 1/1/2026 per IL HB 5395.

Continuation Therapy - Erectile Dysfunction

Covered when ALL of the following are met

Continued Therapy - ED: 1) Member meets one of the following: a) currently receiving medication via Centene benefit or previously met initial approval criteria; OR b) currently receiving medication and enrolled in a state/product with continuity of care regulations; 2) Member is responding positively to therapy; 3) If brand Viagra or Vybrique is requested, member must use generic sildenafil (25 mg, 50 mg, 100 mg) unless contraindicated or clinically significant adverse effects are experienced; 4) Sildenafil (Viagra, Vybrique) is NOT prescribed concurrently with nitrates or guanylate cyclase stimulators; 5) If request is for a dose increase, new dose does not exceed both: a) 100 mg per day; b) Health plan approved quantity limit; Note: Therapeutic failure does not constitute acceptable medical justification. Illinois HIM exception for Vybrique step therapy as of 1/1/2026 per IL HB 5395.

Other uses: 1) If the drug has had a recent label change (within the last 6 months) not yet reflected in this policy, refer to: a) CP.CPA.190 or HIM.PA.33 for drugs on the formulary/PDL; or b) CP.CPA.190 or HIM.PA.103 for drugs NOT on the formulary/PDL. 2) If the requested use (diagnosis, age, dosing) is not listed under section III and criterion 1 does not apply, refer to the off-label use policy: CP.CPA.09 (commercial) or HIM.PA.154 (HIM/ICHRA).

Non‑FDA approved indications for sildenafil that are not specifically addressed in this policy are not authorized unless the provider supplies sufficient documentation of efficacy and safety in accordance with the applicable off‑label use policies (see CP.CPA.09 for commercial lines and HIM.PA.154 for HIM/ICHRA). Prior authorization requests must include clinical documentation demonstrating the requested use meets those off‑label criteria; absent that documentation, the request may be denied.

Therapeutic failure of a preferred or generic product does not by itself constitute acceptable medical justification to bypass the policy requirement that members trial generic sildenafil before receiving brand Viagra or Vybrique. Requests to override the generic‑first step must include documentation of a contraindication or clinically significant adverse effect to generic sildenafil per the policy criteria; otherwise, the generic‑first requirement applies.

Prior Authorization, Documentation, and Denials

Prior Authorization Required

Prior authorization is required. Requests must document that the member has a diagnosis of erectile dysfunction, is ≥ 18 years of age, is not concurrently prescribed nitrates or guanylate cyclase stimulators, and has tried generic sildenafil when brand Viagra or Vybrique is requested unless contraindicated or clinically significant adverse effects are documented.

Prior authorization requires submission of office chart notes, lab results, or other clinical information supporting that member meets all approval criteria.
If brand Viagra or Vybrique is requested, a trial of generic sildenafil (25 mg, 50 mg, 100 mg) is required unless contraindicated or there is documented clinically significant adverse effect.
Therapeutic failure of a non-generic product alone is not acceptable justification to bypass the generic-first requirement.
For Illinois HIM requests, the Vybrique step therapy exception applies as of 01/01/2026 per IL HB 5395 (does not apply to generic-first requirement in other states).
Dose limits: ≤ 100 mg per day and within health plan approved quantity limits.

Step Therapy

Generic-First Requirement

Brand Viagra or Vybrique requests must include documentation of a trial of generic sildenafil unless the provider documents a contraindication or clinically significant adverse effect that prevents use of generic sildenafil.

Generic sildenafil strengths: 25 mg, 50 mg, 100 mg.
Documentation should state reason generic sildenafil is not appropriate if brand is requested.

Documentation Required

Provider must submit supporting documentation with the prior authorization request. Acceptable documentation includes office chart notes, laboratory results, medication history, or other relevant clinical information demonstrating the member meets all applicable criteria.

Requests lacking sufficient clinical documentation will be denied.
Include evidence of current therapy and positive response for continuation requests.

Denial Risk

Requests will be denied if required documentation is missing or if the indication is non‑FDA and lacks sufficient evidence per off‑label use policies CP.CPA.09 (commercial) or HIM.PA.154 (HIM/Marketplace/ICHRA).

Contraindicated in patients with known hypersensitivity to sildenafil or product components.

Background

Sildenafil is a phosphodiesterase‑5 (PDE5) inhibitor approved for the treatment of erectile dysfunction. The usual dose for erectile dysfunction is 50 mg orally approximately 1 hour before sexual activity, with dose adjustments based on response and drug interactions. The policy limits dosing to a maximum of 100 mg per day and recommends lower starting doses when significant drug interactions (for example, strong CYP3A4 inhibitors or ritonavir) are present.
