BCC - Definitive radiation: Member/enrollee is not a surgical candidate, declines surgical resection, or surgery would compromise function or cosmesis due to anatomical location of tumor AND one of the following: low-risk or high-risk BCC; OR advanced BCC (locally advanced, nodal, or metastatic).

Prior authorization is required per the Health Plan for radiation therapy services when applicable. Providers must obtain prior authorization for the radiation therapy CPT/HCPCS codes referenced in this policy before rendering services when the member's benefit plan requires it.

Authorization requirements apply to listed CPT (e.g., 77261–77263, 77280–77295, 77300–77318, 77332–77334, 77385–77386, 77401–77412, 77427, 77470, 77771–77772) and HCPCS/G-codes (e.g., G6001, G6003–G6016) as applicable.

Denial Risk

Not Medically Necessary Modalities

Certain modalities are considered not medically necessary for the treatment of any skin cancer and pose a denial risk if billed. Providers should not expect coverage for these modalities.

Electronic surface brachytherapy — not medically necessary.
Image guided superficial radiation therapy (IGSRT) — not medically necessary.

Providers must document clinical information to support medical necessity for radiation therapy for skin cancer. Complete, contemporaneous medical records should be submitted with authorization requests and upon claim submission to facilitate coverage review.

Tumor histology and diagnosis (e.g., BCC, cSCC, MCC, melanoma) with pathologic confirmation where applicable.
Documentation of surgical candidacy: statement that member is not a surgical candidate, declines surgery, or that surgery would compromise function or cosmesis due to tumor location.
Tumor staging and extent (eg, T stage, nodal involvement, presence of metastasis) and descriptions of local invasion, perineural involvement, or other adverse prognostic features.
Prior treatment history and outcomes (including prior resections, margins status, prior radiation to the field) and rationale if re-irradiation is considered.
Clinical rationale for the selected radiation modality and treatment intent (definitive, adjuvant, palliative) and any multidisciplinary recommendations.

Clinical Background and Rationale

Surgical excision is the primary treatment modality for skin cancers; however, radiation therapy serves as an important alternative or an adjuvant approach when surgery is not feasible, would cause unacceptable functional or cosmetic deficits, or when adverse pathologic features (for example positive margins, extensive perineural invasion, nodal disease, or other high‑risk characteristics) indicate a need to reduce local or regional recurrence risk. Definitive radiation may be used with curative intent for medically inoperable or unresectable disease, and adjuvant radiation is recommended in specified high‑risk postoperative scenarios.
