inv-01: Initial Therapy (PMO)

Covered when ALL of the following are met for postmenopausal osteoporosis (PMO):

Initial Approval Criteria for PMO: 1. Diagnosis of postmenopausal osteoporosis (PMO). 2. Evidence of osteoporosis (meets criteria in policy). 3. Age ≥ 18 years or documentation of closed epiphyses on X‑ray. 4. Failure of a 12‑month trial of an oral bisphosphonate (see Appendix B; generic alendronate preferred) at up to maximally indicated doses, unless contraindicated or clinically significant adverse effects are experienced. 5. Dose does not exceed both of the following: (a) 3 mg every 3 months; (b) 1 syringe every 3 months. 6. Member must use generic ibandronate injection unless contraindicated or clinically significant adverse effects are experienced.

Provider must submit documentation (office chart notes, lab results, or other clinical information) supporting that member has met all approval criteria.

Continuation/Renewal criteria (when meeting initial criteria previously):

Continuation Therapy: 1. Member is currently receiving ibandronate injection via the plan benefit or previously met initial approval criteria. 2. Member is responding positively to therapy. 3. Member must use generic ibandronate injection unless contraindicated or clinically significant adverse effects are experienced. 4. If request is for a dose increase, the new dose must not exceed both: (a) 3 mg every 3 months; (b) 1 syringe every 3 months.

Provider must submit documentation (office chart notes, lab results, or other clinical information) supporting continued medical necessity. Approval durations differ by line of business (Medicaid/HIM 12 months; Commercial 6 months or to renewal date).

Non‑FDA approved indications for ibandronate injection are not covered under this clinical policy unless the use is specifically addressed by the plan's off‑label use policies or by the member's Evidence of Coverage. Providers requesting coverage for an off‑label indication must follow the referenced off‑label policies: CP.CPA.09 (commercial), HIM.PA.154 (health insurance marketplace), or CP PMN.53 (Medicaid), and submit supporting documentation demonstrating efficacy and safety consistent with those policies.

For members enrolled in Medicaid, state Medicaid coverage provisions take precedence when they conflict with the coverage provisions in this clinical policy. Providers should consult the applicable state Medicaid manual and follow any state‑specific guidance that supersedes this policy.

Requests for ibandronate injection to treat non‑FDA approved conditions will not be authorized unless the request includes sufficient clinical documentation as required by the applicable off‑label use policy (see CP.CPA.09, HIM.PA.154, or CP PMN.53) or other plan Evidence of Coverage demonstrating the medication’s efficacy and safety for the requested indication.
