Coverage Criteria for Eptinezumab (Vyepti)

Continuation Therapy

Covered when ANY of the following continuity criteria are met

Continued therapy: 1) Member is currently receiving Vyepti via the Centene benefit or previously met initial approval criteria; 2) Provider attestation of maintained positive response to therapy (reduction in monthly migraine days from baseline); 3) Vyepti is not prescribed concurrently with other CGRP inhibitors; exception: California (CA) continuity exception applies if member was previously approved via Centene benefit and is currently stable on therapy with both oral and injectable (and per later updates, other) CGRP inhibitorssee text
