Coverage Criteria for Ribavirin

Initial Therapy — Hepatitis C Infection

Covered when ALL of the following are met:

Initial Hepatitis C criteria: 1. Diagnosis of hepatitis C virus (HCV) infection as evidenced by detectable serum HCV RNA levels by quantitative assay in the last 6 months; 2. Member meets prior authorization criteria for Epclusa, Harvoni, Mavyret, Sovaldi, Zepatier, Viekira Pak, or Vosevi for combination use; 3. Member meets one of the following (a or b): a. Ribavirin tablet: Age ≥ 5 years; b. Ribavirin capsule: Age ≥ 3 years; 4. Dose does not exceed the following (a or b): a. Ribavirin tablet: 1,200 mg per day; b. Ribavirin capsule: 1,400 mg per day.see node text

Approval duration coincides with duration for Epclusa, Harvoni, Mavyret, Sovaldi, Zepatier, Viekira Pak, or Vosevi authorization

Continuation Therapy — Hepatitis C Infection

Covered for continuation when ALL of the following are met:

Continued Hepatitis C criteria: 1. Member meets one of the following (a or b): a. Currently receiving medication via Centene benefit or member has previously met initial approval criteria; b. Member is currently receiving medication and is enrolled in a state and product with continuity of care regulations; 2. Member is responding positively to therapy; 3. If request is for a dose increase, new dose does not exceed the following (a or b): a. Ribavirin tablet: 1,200 mg per day; b. Ribavirin capsule: 1,400 mg per day.see node text

Non–FDA‑approved indications that are not specifically addressed in this policy are not authorized unless the provider supplies sufficient documentation of efficacy and safety in accordance with the applicable off‑label use policies (see CP.CPA.09 for Commercial, HIM.PA.154 for Health Insurance Marketplace, or CP.PMN.53 for Medicaid) or other evidence of coverage documents.

This policy includes several substantive revisions effective with the 4Q 2024 update: removal of references to branded ribavirin products (including Rebetol and Ribasphere Ribapak) because those brands are no longer available; elimination of the specialist prescriber requirement and the accompanying Appendix E; and removal of the requirement that the hepatitis C diagnosis be specified as chronic, allowing use for acute HCV where aligned with AASLD guidance.

Requests for ribavirin to treat indications that are off‑label and not covered by this policy will be evaluated under the applicable off‑label use policies. If adequate clinical evidence and safety data are not submitted consistent with those off‑label policies, the use will be considered not authorized (not medically necessary).

Approval duration for ribavirin will coincide with the authorization period for the listed direct-acting antiviral regimens (Epclusa, Harvoni, Mavyret, Sovaldi, Zepatier, Viekira Pak, or Vosevi) when used in combination.

Background and Drug Information

Ribavirin is a guanosine nucleoside analogue used as part of combination antiviral regimens for hepatitis C virus infection. It is administered orally as tablets or capsules with pediatric age indications and dosing individualized by genotype, age, treatment response, and tolerability. Note that branded ribavirin products are no longer commercially available per the policy revision history.
