General coverage and coding rules

General coding and claims payment rules, exclusions, and editing behaviors that affect reimbursement.

Services are subject to the member's benefit plan documents and applicable member cost-sharing; verify eligibility and benefits prior to services.

Only standard diagnosis and procedure codes that comply with HIPAA transaction code set standards are accepted.

Claims will be screened for conflicts with core data. Reimbursement will not be made when procedure or diagnosis codes conflict with common core data such as place of service, patient age, diagnosis/procedure mismatches, or provider/procedure conflicts.

Procedures that are incidental to a primary procedure (included in the primary procedure by clinical practice standards) are not reimbursed separately.

Mutually exclusive procedures that cannot reasonably be performed at the same anatomic site or patient encounter are considered submitted in error; only the primary service is considered for reimbursement.

Codes described as 'separate procedure' may be eligible for separate reimbursement only when performed in a different session or at an anatomically unrelated site; append XE or XS when supported by documentation.

Unbundling (reporting multiple codes when a single comprehensive code applies) may result in denial or re-bundling and processing as the accurate comprehensive code.

Bilateral services reimbursement

When a procedure is performed bilaterally during the same session or on the same day, report the procedure on one line with modifier 50 as defined by CPT.

Do not use LT/RT when modifier 50 applies; use LT/RT only when the procedure was performed on one side and modifier 50 is not appropriate.

Bilateral services billed with modifier 50 are reimbursed at 150% of the fee schedule allowed amount.

Modifiers that reduce the allowed amount must be billed in the primary modifier position and modifier 50 in the secondary position when applicable (e.g., bill modifier 26 primary and modifier 50 secondary for professional component of a bilateral procedure).

Point32Health will not accept medical record documentation or physician queries amended more than 30 days from the date of service to defend or increase reimbursement or for consideration of a previously denied claim.
