Covered when provided by contracted, credentialed ABA providers who meet staffing, documentation, and service‑delivery requirements and when services are medically necessary as supported by assessment:

ABA service coverage preconditions: Services must be delivered by a Licensed Applied Behavior Analyst (LABA) or a team including a LABA and Behavior Technician (BT); provider must maintain infrastructure for quality management/assurance, utilization management, electronic data collection/IT, clinical/psychiatric expertise, and cultural and linguistic competence.

Assessment and planning: LABA completes a written comprehensive assessment, inclusive of a Functional Behavior Assessment (FBA) when applicable, and develops an individualized treatment plan with specific, measurable objectives, caregiver goals, monitoring, and transition plans.

Service setting and justification: Treatment location must be determined by medical necessity; services are delivered primarily in home and/or community. Center‑based services may be authorized only when clinically indicated with documented clinical rationale, safety/generalization plans, and transition plans back to natural settings; school‑based services are distinct and typically the responsibility of the educational system (IEP/IFSP).

Caregiver training and group instruction: Treatment plans must include at least two specific, measurable caregiver goals and prescribe parent/caregiver training no less than one hour per month; group parent/caregiver training must align with a curriculum and be linked to Member goals, with group size and billing limits observed.>= 1 hour/month

Services provided in school settings are distinct and separate from health plan–covered ABA services and are typically the responsibility of the educational system through the Individualized Education Program (IEP). Providers and clinical teams should therefore not duplicate school-based services when determining medical necessity or authorizing ABA under the health plan. When ABA services are needed outside the school setting, coverage applies only for services delivered by contracted, credentialed ABA providers in settings such as the member’s home, community, or an approved center, consistent with the policy’s service-setting and documentation requirements.

Initial authorization and assessment must be completed and services started without delay. Following initial assessment authorization, the ABA provider must ensure there is no delay in the start of services. Once an initial authorization is approved, the ABA provider will, within two business days, offer a face-to-face interview with the Member's family. The LABA must complete the required written comprehensive assessment (including an FBA/FA as applicable) that supports the need for ABA prior to initiating the individualized treatment plan and supervised services.

Providers must make best efforts to initiate services within the 14-calendar-day timely-provision standard. If services cannot be initiated within 14 days of authorization (or within 14 days of initial contact if authorization is not required), the provider must maintain a waitlist, offer caregivers contact information for alternative providers accepting new clients, and refer the Member to local CBHC, Behavioral Health Help Line, and/or the Managed Care Entity for assistance. DefinitionFunctional Behavior Assessment (FBA) is a descriptive and systematic behavioral assessment developed by the LABA that identifies functions of behavior and includes behavior analytic interpretation of results; when severe behaviors are present this may include a functional analysis (FA) in a controlled environment.

PurposeTo identify antecedents, consequences, and functions maintaining problem behavior to inform individualized behavior analytic interventions and treatment planning.

Role in treatmentThe LABA develops the FBA, uses findings to design interventions and behavior plans, and documents results in the treatment record to support measurable objectives and safety planning.

Operational limits: Documentation should address appropriateness for each component delivered via telehealth; telehealth is an adjunct modality and cannot be substituted for required in‑person services when clinically necessary.
