For opioid requests, provide the daily cumulative Morphine Equivalent Dose (MED), indicate whether you are aware of other opioid prescribers for the enrollee (and identify them if so), and justify that the stated daily MED is medically necessary and that a lower total daily MED dose would be insufficient to control the enrollee's pain.
