Defines billing and reimbursement rules for HCPCS code Q3014 (telehealth originating site facility fee) for Florida Blue across all lines of business when services are billed on CMS-1500 or UB-04 forms and where the facility is the patient's physical location during the telemedicine encounter.

Q3014 reimbursement criteria
Covered when ALL of the following are met:

- Q3014 must be submitted by the originating site (facility) when providing administrative and clinical support services for a patient receiving services via telemedicine.
- The encounter must occur in an outpatient medical facility; Q3014 is not reimbursable for encounters which occur outside of a clinical setting.
- Q3014 may be reported when the distant provider bills POS 02, 19, 22, 31, 53, or 72; Q3014 may NOT be reported if the distant provider billed POS 10 or 11.
- Q3014 must be billed on the same date of service as the corresponding professional telemedicine claim and must indicate the physical location of the facility where the member was located during the encounter.
