Admission meets inpatient criteria when the patient is in the facility overnight and the admission and discharge dates span at least two consecutive dates (i.e., at least two calendar dates).

Exceptions that allow inpatient billing with same admission and discharge date

Patient was transferred to another hospital or facility; such a transfer counts as one inpatient day.
Patient expired (died) during the admission; the admission counts as one inpatient day.
Patient left against medical advice after occupying an assigned inpatient bed; this counts as one inpatient day.

Claims with the same admission and discharge date should be billed as outpatient/observation except for the exceptions listed above.

Observation services may span two or more consecutive dates and remain outpatient; observation is not inpatient even if it spans multiple dates.

All inpatient admissions require authorization or notification as dictated by plan language. If a member was authorized as inpatient but did not stay overnight, the authorization should be updated to outpatient/observation within 7 days of the event.

The discharge date is not included in length-of-stay calculation unless the patient died, was transferred, or left against medical advice. Interim bills may include the discharge date in length-of-stay calculation.

If authorization is requested for an inpatient admission and criteria are not met, the authorization review will advise that observation/outpatient status applies rather than inpatient.

This update process is the same when a patient is initially outpatient and then converted to inpatient — the authorization must be revised to accurately reflect the level of care.

Florida Blue reviews inpatient authorization requests to determine whether the admission meets inpatient criteria. If the requested inpatient admission does not meet inpatient criteria, the hospital will be advised that observation status (outpatient) applies for the services rather than inpatient admission. Authorized inpatient claims will be honored when the member was inpatient overnight for at least two consecutive days, or in exceptions such as transfer to another facility, death on the date of admission, or leaving against medical advice after occupying an assigned inpatient bed — each of which may count as one inpatient day.
