Outpatient related services billing criteria

Outpatient related services should be billed as a single claim when they meet the policy's related-service timeframes or continuity criteria; series billing and certain pre-operative testing have specific guidance.

Outpatient services performed for the same or a related condition should be submitted as a single claim unless documentation supports separate or unrelated episodes of care.

Services that occur within 3 days (lookback/lookforward) of another outpatient claim for treatment should be billed as a single claim.
When outpatient services are performed continuously without the patient being discharged (for example, transfer from ER to observation spanning multiple days), submit as a single claim.

Pre-operative testing

Pre-operative testing performed within 7 days of an outpatient surgical claim should be billed on the outpatient surgical claim.
Revenue codes often considered pre-operative testing include laboratory/urinalysis (032X), radiology diagnostic (073X), and EKG/ECG when billed by themselves or together.

Diagnostic imaging aggregation

If multiple diagnostic imaging services are performed within a 72-hour period for the same condition or the same anatomical region, submit those imaging services on a single claim.

Series billed services: Series billing (chemotherapy, radiation, infusion, PT/OT/ST, cardiac rehabilitation, hyperbaric oxygen, behavioral health) should continue based on historical practices (weekly, bi-weekly, or monthly) and, if other outpatient services occur on the same day as a series billed service, submit both on the same claim.

Adjudication and denial process

When adjudicating an outpatient claim, Florida Blue will check claim history 3 days prior to the claim's 'from date' and 3 days from the claim's 'through date' to identify related reimbursed outpatient claims.
For pre-operative testing the lookback is 7 days when checking for related surgical claims.
If the outpatient claim under adjudication is determined to be related to a prior reimbursed outpatient claim, the second claim will be denied indicating a single bill is required.
