Reimbursement Eligibility
Reimbursement criteria
Reimbursement is provided when the basic office service is eligible for reimbursement and billed appropriately.
ALL of the following
ONE OF
- CPT codes 99050 or 99051 are reported in addition to the basic office service when provided in an office setting during after‑hours, evening, weekend, or holiday hours (evening hours defined as after 5:00 p.m.).evening hours = after 5:00 p.m.
Applies only when the basic service is eligible for reimbursement.
- Billing is submitted on a CMS‑1500 or equivalent claim form.
Same provider includes all physicians and/or other health care professionals reporting under the same Federal Tax Identification number.
