GLP-1 prior authorization for commercial plans
Defines Florida Blue commercial plan coverage and prior authorization requirements for GLP-1 medications, limiting coverage to treatment of type 2 diabetes and outlining submission instructions for providers and pharmacies.
Effective April 1, 2025, members with a Florida Blue commercial plan currently on a prescribed GLP-1 therapy will be required to have a prior authorization on file from a provider.
Florida Blue commercial plans only cover GLP-1 drugs when prescribed for the treatment of type 2 diabetes; off-label indications are not covered.
Preferred GLP-1 drugs listed for type 2 diabetes coverage: Mounjaro, Ozempic, Rybelsus.
Medications used for weight loss/management (e.g., Wegovy, Zepbound, Saxenda) are excluded from coverage.
