Effective April 1, 2025, members with a Florida Blue commercial plan currently on a prescribed GLP-1 therapy will be required to have a prior authorization on file from a provider.

Florida Blue commercial plans only cover GLP-1 drugs when prescribed for the treatment of type 2 diabetes; off-label indications are not covered.

Incomplete or multiple submissions may delay review; respond to requests for additional information (48-hour response for urgent pharmacy requests)

Medications prescribed for weight loss/management (for example, Wegovy, Zepbound, and Saxenda) are excluded from coverage under Florida Blue commercial plans. Prior authorization requests submitted for these indications will be denied and are the patient’s financial responsibility.

Use of GLP-1 receptor agonists for off-label indications, including treatment for weight loss or weight management, is not covered and will be considered not medically necessary. Authorization requests for non–type 2 diabetes indications will be denied without clinical documentation supporting an approved indication.
