Require physician or licensed independent practitioner documentation including a written order specifying “Admit to Observation.” Report G0378 (hospital observation service, per hour) with units equal to hours of observation (rounded to the nearest hour) on a single line with the date of service equal to the observation start. Use G0379 for direct admissions to observation when the patient is referred directly to observation after evaluation by a community physician.
