Medical Necessity and Coverage Criteria

Initial facility-based PSG

Covered when ALL of the following are met:

Initial PSG criteria: Member/enrollee is ≥18 years and has suspected OSA

based on I.A

Repeat facility-based PSG

Covered when ALL of the following are met:

Repeat PSG base criteria: All criteria in Initial PSG (section I.B) are met

Section II.A

Repeat frequency limit: Requested study and any previous studies amount to two or less per rolling year<=2 per rolling year

Indications for repeat PSG: One or more of: oral appliance adjusted and requires efficacy assessment; change of device due to intolerance; assessment of PAP settings due to continued symptoms despite adherent use (at least 4 hours/night for 70% of nights over a 30-day period); significant weight loss (>=10%) to consider discontinuation of PAP; significant weight gain (>=10%) or recurrent symptoms; postoperative assessment after upper airway surgery; remote history of OSA not on PAP needing re-establishment of diagnosis/initiation of CPAP; new signs/symptoms after prior negative study

Section V and coding

Actigraphy testing performed and reported under CPT 95803 is considered insufficient evidence when used alone to diagnose obstructive sleep apnea (OSA). The policy does not support coverage for CPT 95803 for the purpose of diagnosing OSA because actigraphy measures activity patterns rather than the physiologic respiratory parameters required for a definitive OSA diagnosis.

Coverage of services described in this policy is subject to the terms, conditions, exclusions, and limitations of the member's coverage documents (for example, evidence of coverage, certificate of coverage, policy or contract). Where applicable, state Medicaid provisions take precedence over this clinical policy. Providers should verify member benefits and obtain any required authorizations per plan procedures before scheduling services.

Use of actigraphy as a standalone diagnostic modality for obstructive sleep apnea is not supported by the evidence and is therefore not covered under this policy. Actigraphy does not provide the comprehensive physiologic respiratory monitoring achieved with polysomnography or appropriately equipped home sleep apnea testing devices and cannot reliably establish the diagnosis of OSA on its own.

Facility-based polysomnography (PSG), split-night studies, and facility-based CPAP/BiPAP titration are considered medically necessary only when the specific clinical criteria in this policy are met. For initial or repeat facility-based PSG/split-night studies the member must have suspected obstructive sleep apnea (OSA) and one or more reasons why home sleep apnea testing (HSAT) is not appropriate (for example: HSAT unavailable, inability to operate HSAT equipment, prior inadequate HSAT results, chronic opioid use, or low pretest probability scenarios where PSG sensitivity is higher). Repeat PSG/split-night studies are limited to situations meeting the repeat-study criteria (including indications such as device intolerance, need to adjust therapy, significant weight change, postoperative assessment, or renewed symptoms) and are limited to two or fewer studies per rolling year.

Initial and repeat facility-based PSG or split-night study: Member must have suspected OSA and meet the listed indications where HSAT is not appropriate.
Repeat studies: All initial criteria plus one of the repeat indications and ≤ 2 studies per rolling year.

Clinical Background

Obstructive sleep apnea (OSA) is characterized by recurrent upper-airway collapse during sleep that can cause excessive daytime sleepiness and increase cardiometabolic risk. The gold-standard diagnostic test is facility-based polysomnography (PSG), which records multiple physiologic parameters necessary to identify and stage sleep-disordered breathing. Home sleep apnea testing (HSAT) can be an alternative for select patients at high pretest probability for moderate to severe OSA and without significant comorbidities, but HSAT measures fewer physiologic variables than PSG and may under-detect events in some populations. For certain interventions such as hypoglossal nerve stimulation, facility-based titration PSG performed approximately 3–6 months after device activation is the standard of care to optimize device settings.
