Initial Therapy / Diagnostic and Stabilization Injections

Covered when ALL of the following are met:

Initial/diagnostic injections: 1) Myofascial pain persisting >3 months causing tenderness and/or weakness, restricting motion and/or causing referred pain when compressed; 2) Failed ≥3 weeks of conventional multidisciplinary medical therapy including all: (i) chiropractic, physical therapy, or prescribed home exercise program or intolerance to such therapy when injection is a bridge; (ii) NSAID unless contraindicated or not tolerated; (iii) activity modification; 3) Trigger points identified by palpation; 4) Trigger points are located in a few discrete areas and are not associated with widespread muscle tenderness (eg, not fibromyalgia); 5) Injections are adjunctive (not sole therapy) to facilitate mobilization to allow non-invasive modalities (eg, physical therapy).Up to two sets of injections at least seven days apart for the same trigger point; a body region injection counts as one service regardless of number of injections.

Procedure must be physician-performed; only one procedure per visit.

Continuation Therapy / Repeat Injections

Additional injections (up to four) are medically necessary when ALL of the following are met:

Additional injections: A) Prior injections resulted in ≥ 50% pain relief with functional improvement for ≥ 6 weeks; B) There was return of pain and/or deterioration following ≥ 6 weeks of improvement; C) Injections are given at least two months apart for up to 12 months from the initial injection; D) Injections are adjunctive (not sole therapy) to facilitate mobilization to allow non-invasive modalities (eg, physical therapy).Maximum six total sessions from initial injection; injections at least 2 months apart; additional up to four sessions when criteria met.

Unsupported or Not Supported by Current Evidence

Current evidence does not support the following indications:

Unsupported therapies: A) Dry needle stimulation of trigger points; B) Trigger point injection with saline or glucose; C) Use of Botox during trigger point injections.

The policy specifies that current evidence does not support several trigger point therapies. In particular, dry needle stimulation (dry needling) of trigger points and trigger point injections using saline or glucose are not supported by available scientific data and therefore are excluded from medical necessity under this policy. Additionally, the use of botulinum toxin (Botox) during trigger point injections is not supported by current evidence. Claims submitted with procedure codes that represent needle insertion without injection (e.g., CPT 20560 and 20561) do not meet the coverage criteria for trigger point injection services and are at risk for denial.

Because the literature contains limited and inconsistent data showing patient outcome benefit, the policy states these specific interventions are not recommended based on available evidence. Providers should not rely on dry needling, saline/glucose injections, or Botox used during trigger point procedures to meet the clinical criteria for covered trigger point injections under this policy.

Conservative Therapy Prerequisite

Providers must document that the member has failed at least 3 weeks of conservative multidisciplinary therapy prior to initial trigger point injections, unless the member is unable to tolerate or has contraindications to such therapies. Conservative therapies include chiropractic care, physical therapy, or a prescribed home exercise program; NSAIDs (unless contraindicated); and activity modification.
