Do not use non-supported CPT codes for coverage

CPT codes 64451 and 64625 are not supported for coverage under this policy and should not be billed as covered SIJ joint injection procedures for anesthetic/steroid injections or radiofrequency ablation/neurotomy of the SIJ.

Requests beyond 12 months require supporting evidence

For requests to continue SIJ injections beyond 12 months, provide documentation of sustained benefit (at least 50% sustained improvement in pain and/or 50% sustained objective improvement in function using the same baseline scale for at least three months), measures demonstrating functional disability or effect on ADLs, and a rationale for continuation (for example, high surgical risk or patient declines surgery).
