Coverage and Medical Necessity Criteria

Primary bariatric surgery — Medical necessity

Covered when ALL of the following are met per policy and referenced MCG criteria.

Primary bariatric surgery general

BPD-DS specific criteria: 1) Age ≥ 18; 2) Severe obesity (eg, BMI of 50 kg/m² or greater); 3) Patient has tried and has failed to achieve and maintain sufficient weight loss with nonsurgical treatment; 4) Correctable cause for obesity not identified (eg, hypothyroidism, Cushing syndrome); 5) Current substance abuse not identified; 6) Not currently pregnant and no planned pregnancy within 18 months of surgery; 7) Expectation that patient will be able to adhere to postoperative care requirements (eg, judged to be committed, and willing to participate and adhere to postoperative instructions); 8) No current untreated eating disorder; 9) No serious untreated or uncontrolled medical, psychiatric, psychosocial, or cognitive condition that would interfere with adherence to postoperative instructions, and self-management; 10) Patient is receiving treatment in multidisciplinary program experienced in metabolic surgery that can provide appropriate care (eg, preoperative medical or mental health consultation, nutrition counseling, patient support programs).

Repeat/revision bariatric surgery — Medical necessity

Repeat or conversion bariatric surgery: Covered when ALL of the following apply for revision/conversion due to failure or complication.

Repeat/Conversion criteria: A) To correct complications from a previous bariatric surgery (e.g., obstruction or strictures) including conversion surgeries to SG, RYGB (adults/adolescents) or BPD-DS (adults); OR B) Conversion from LAGB to SG, RYGB, or BPD-DS; or revision for dilation of gastric pouch when all of the following are met: 1) All criteria listed in MCG current edition for the initial bariatric procedure are met again; 2) Previous surgery for morbid obesity was at least two years prior to repeat procedure; 3) Weight loss from the initial procedure was less than 50% of the member/enrollee's excess body weight at the time of the initial procedure; 4) If the conversion is requested due to removal of an eroded laparoscopic adjustable band, at least two months have passed between the band removal and the subsequent bariatric procedure; 5) Documented compliance with previously prescribed postoperative nutrition and exercise program; 6) Supporting documentation must include a clinical explanation of the circumstances as to why the procedure failed;

The following procedures are considered investigational due to inadequate evidence and are not supported as standard bariatric interventions: distal (very long‑limb) gastric bypass, mini/one‑anastomosis (mini) gastric bypass, laparoscopic re‑sleeve gastrectomy (LRSG), Fobi pouch, laparoscopic greater curvature plication (gastric imbrication), LAP‑BAND when BMI 30–35 kg/m², stomach aspiration therapy (AspireAssist), endoscopic suture revisions post‑bariatric surgery, single anastomosis duodenoileal bypass (SADI), gastric plication/endoluminal vertical gastroplasty, endoscopic gastrointestinal bypass devices (EGIBD), endoscopic sleeve gastroplasty, transoral endoscopic surgery, and vagus nerve blocking (e.g., Maestro).

Certain historical or largely abandoned procedures are explicitly identified as not supported for contemporary bariatric care: jejunoileal bypass (JIB) (no longer recommended due to high complication rates), vertical banded gastroplasty (VBG) (largely superseded by adjustable gastric banding), older generation gastric balloons (removed from the market for safety concerns), and gastric pacing (no demonstrated long‑term benefit and abandoned). These are noted as obsolete or not routinely performed.

The following procedures are listed as not medically necessary because of unfavorable risk‑benefit profiles or insufficient evidence of durable benefit: biliopancreatic diversion (BPD/Scopinaro), jejunoileal bypass (JIB), vertical banded gastroplasty (VBG), gastric pacing/gastric electrical stimulation, and gastric wrapping. BPD variants are distinguished in the policy (see exclusions for BPD‑DS criteria and BMI‑related considerations).

Procedures and related services for which the policy does not support medical necessity include placement or removal of intragastric bariatric balloons and implantation, revision, or removal of gastric neurostimulator systems; these services are represented by CPT codes that the policy identifies as not supporting medical necessity (for example, CPT codes 43290, 43291, 43632, 43647, 43648, 43842, 43881, 43882, 64590, 64595). Providers should expect claims for these codes to be at risk for denial when submitted without supporting indications recognized by this policy.

State Medicaid coverage provisions and applicable Medicare National Coverage Determinations (NCDs) or Local Coverage Determinations (LCDs) take precedence over this clinical policy when there is a conflict. Repeat procedures (except removal/adjustment of port or device) generally require: meeting initial MCG criteria again; previous bariatric surgery ≥ 2 years prior; prior weight loss < 50% of excess body weight after initial procedure; documented compliance with postoperative nutrition and exercise; and, when applicable, at least 2 months between band removal and subsequent procedure.
