Code and Clinical Editing Guidelines — Professional and Facility
Governs the health plan's application of code and clinical editing guidelines (CCEG) to evaluate and adjudicate professional and facility claims for accuracy and coding/billing compliance; affects providers submitting claims to Empire Bluecross commercial plans.
Previously separate professional policy (C-09004) was retired and combined into a new blended policy titled Code and Clinical Editing—Professional and Facility (C-25002).
CCEG measures are updated and claims submitted after configuration implementation will be processed according to updated CCEG measures without retrospective payment changes for edits applied by module updates.

Coverage and reimbursement are subject to code edits and plan benefits; specific rules:
ALL of the following
- Claims are evaluated using Code and Clinical Editing Guidelines (CCEG) which include code editing software, CMS NCCI edits, Outpatient Code Edits (OCE), PTP logic, clinical criteria, MUEs, claims processing platform, and Uniform Billing Editor.
- CCEG measures are updated as applicable to incorporate new codes, code definition changes, and edit rule changes; claims submitted after a CCEG configuration implementation date will be processed under the updated measures and no retrospective payment changes, adjustments, or refund requests will be made due to module updates.
AND the following plan/contract conditions apply
- Reimbursement is subject to applicable provider, state, and federal contracts or mandates that may supersede CCEG application.
