CCEG reimbursement criteria

Coverage and reimbursement are subject to code edits and plan benefits; specific rules:

ALL of the following

Claims are evaluated using Code and Clinical Editing Guidelines (CCEG) which include code editing software, CMS NCCI edits, Outpatient Code Edits (OCE), PTP logic, clinical criteria, MUEs, claims processing platform, and Uniform Billing Editor.
CCEG measures are updated as applicable to incorporate new codes, code definition changes, and edit rule changes; claims submitted after a CCEG configuration implementation date will be processed under the updated measures and no retrospective payment changes, adjustments, or refund requests will be made due to module updates.
AND the following plan/contract conditions apply
- Reimbursement is subject to applicable provider, state, and federal contracts or mandates that may supersede CCEG application.

Billing Rule

Submit accurate CPT/HCPCS/revenue codes and meet authorization/medical necessity

Use correct CPT, HCPCS, and revenue codes on all claims and ensure the codes are fully supported in the medical record or office notes; services must also meet authorization and medical necessity guidelines appropriate to the procedure, diagnosis, and member's state of residence. Follow industry-standard billing and submission guidelines; failure to comply may result in claim rejection or recovery of payment.

Bill services with CPT and HCPCS codes (and revenue codes on facility claims) supported by the medical record.
Ensure services meet authorization and medical necessity requirements applicable to the procedure, diagnosis, and member's state of residence.
Use industry-standard compliant codes and submission practices to avoid claim issues.

Denial Risk

Denials and recovery for MUE and CCEG conflicts

Claims with units that exceed Medically Unlikely Edits (MUEs) or that conflict with CCEG will be denied and not reimbursed; the plan may recover or recoup payments for claims that do not follow billing guidelines.

When a service unit exceeds an MUE, the claim line(s) will be denied.
If a service conflicts with CCEG, the Health Plan will not reimburse the service.
The plan may recover and/or recoup claim payment or reject/deny the claim if coding/billing guidelines are not followed.
