Specifically, they should not be used as an athletic mouth guard, for treatment of temporomandibular disorders or myofascial pain dysfunction, as an appliance for orthodontic tooth movement, for treatment of obstructive sleep apnea, or for procedures such as tooth whitening, fluoride delivery, or delivery of periodontal medicaments.

Follow administrative and billing instructions exactly as specified by the payer. Submit accurate CDT/CPT/ICD-10 codes, attach required documentation, and bill only for services that are covered under the member's contract.

Use the current CDT code set for dental procedure coding and the current ICD-10-CM for diagnoses.
Include itemized documentation for services such as fabrication (D9944/D9945/D9946) and adjustments/repairs (D9942/D9943).

Narrative Requirement

A narrative explaining the clinical rationale for the occlusal guard is required when indicated. The narrative should describe the diagnosis, clinical findings, treatment goal, and why the appliance is necessary for this patient.

Include justification for use (e.g., protection of natural teeth from opposing porcelain restorations, relief of occlusal stress post-periodontal therapy, or management of bruxism).
Attach supporting records such as diagnostic casts (D0470), imaging (D0330), and prior treatments.
