Submit current (within 12 months) dated periodontal chart documenting millimeters of recession (CEJ to gingival margin), millimeters of attached gingiva, and 6-point pocket depth measurements; document history of progressive recession within 12 months, notation of frenum involvement, number of teeth affected, photographic documentation of areas demonstrating recession as needed, current (within 12 months) pretreatment radiographs as needed, indication of root sensitivity, a narrative rationale addressing status of keratinized attached gingiva, root coverage, sensitivity and/or cosmetics, and relationship to cervical caries/existing restorations.

Procedure inclusivity

Frenectomy or frenuloplasty performed on the same date and in the same area as a soft tissue graft is considered inclusive and is not separately benefitted.

Denial Risk

Not covered when good oral health/no plaque buildup

Mucogingival surgery and grafting are inappropriate and may be denied in the presence of good oral health where no plaque buildup is evident.

Denial rationale: absence of plaque buildup/good oral health means procedures are not necessary and may be denied.

Billing Rule

Biologic materials not benefitted

Biological materials (for example, dermal matrix materials) are not benefitted when submitted in conjunction with soft tissue grafting.
