Initial/Emergency Treatment

Covered when ALL of the following are met:

Injury type: Physical damage or injury from extra-oral blunt force trauma to sound natural teeth and/or supporting hard and soft tissue structures (not due to chewing or biting forces).

Sound natural teeth are defined as stable, functional, and free from decay, fracture, and advanced periodontal disease at the time of the accident.

Timing: Treatment rendered within seventy-two (72) hours of onset of injury unless delayed due to hospitalization for other accident-related injuries.<=72 hours (unless hospitalized)

Initial/emergency care timeframe.

Standards of care: Services are in accordance with generally accepted standards of dental practice and are effective, necessary, not primarily for convenience, and not cosmetic.

Care evaluated against prevailing standards of dental practice.

Permitted initial/emergency services: Initial or emergency treatments appropriate to the injury may include exam and diagnosis; radiographic exam; extraction; suturing; splinting; re-implantation, repositioning, and stabilization of dislodged teeth; restorative services; endodontic services; interim prosthetic services; and medication administered by the provider.

Definitive Restorative/Reconstructive Treatment

Covered when ALL of the following are met:

Timing: Definitive restorative and reconstructive treatments are performed within 12 months of the date of the accidental dental injury.<=12 months

Timeframe for definitive treatment.

Service types: When performed within the timeframe, definitive services may include extraction; endodontic services; periodontic services including soft tissue grafting; restorative services; implant services; fixed and removable prosthodontic services; and bone grafting.

Standards of care: Definitive treatments must be in accordance with generally accepted standards of dental practice and be effective, necessary, not primarily for convenience, and not cosmetic.

Clinical appropriateness requirement for definitive care.

Services that treat dental conditions which existed prior to the accident are not considered appropriate for coverage by the plan unless the group contract explicitly states otherwise. This means restorative, periodontal, endodontic, prosthodontic, or other dental work performed to address pre‑existing disease, decay, or damage will generally be excluded from payment when submitted as treatment for an accidental dental injury unless contract language provides coverage.

When evaluating claims, reviewers should confirm that the procedures billed directly address injury‑related damage rather than chronic or pre‑existing conditions; absence of documentation linking treatment to the accident increases the risk of denial.

If a service primarily addresses a pre‑existing dental condition rather than damage caused by the accidental event, it is considered not appropriate and may be denied unless the member’s group contract specifically allows coverage. Providers should not characterize elective or pre‑existing care as part of accidental injury treatment to obtain payment.

Documented evidence that procedures were necessitated by the accident (for example, notes describing acute trauma findings, dated radiographs or photos showing new injury, and timing consistent with the event) is required to support a claim that otherwise might appear to treat a pre‑existing condition.

Services for pre-existing or pre-accident dental conditions are not considered appropriate for treatment of accidental dental injuries unless the member's contract specifically provides coverage for such services. When any service is performed in conjunction with or in preparation for a non-covered or denied service, all related services may also be non-covered or denied. Providers should verify member contract benefits prior to rendering non-emergent care to avoid denial risk.

Documentation Required

Required clinical documentation

Diagnostic and clinical documentation is required to support emergency and definitive treatment of accidental dental injuries. Documentation must be clear, dated, and of diagnostic quality to permit retrospective review.

Diagnostic pre- and post-trauma radiographs (properly oriented, labeled, and dated) — may include full mouth series, panoramic, or other appropriate radiographic images showing all existing and missing teeth in both upper and lower arches
Chart notes describing the accident, findings, treatments rendered, and proposed treatment plan
Intra-oral and facial photographs when appropriate
Accident or emergency room report and a letter from the treating dentist describing the accident and dental/oral injuries
