Meets coverage when testing to identify bacteria and antibiotic susceptibilities for a urinary tract infection (UTI).

Meets coverage for asymptomatic individuals undergoing urological interventions that breach the mucosa (pre-procedure testing).

Meets coverage for individuals exhibiting at least one sign or symptom of possible UTI or bacteriuria (e.g., fever, dysuria, urgency, frequency, pyuria, suprapubic tenderness, flank pain, hematuria, cloudy urine, nausea, vomiting, chills, change in urine smell).

Meets coverage to assess pyelonephritis (including isolate identification and susceptibilities if applicable).

Do not use urine culture to screen asymptomatic catheterized patients as the sole indicator for catheter-associated UTI; do not screen/treat ASB in long-term care residents and many catheterized patients per guideline recommendations.

Generally recommend against routine collection or treatment of bacteriuria in asymptomatic kidney transplant recipients more than two months post-transplant; if screening earlier or positive, obtain a second culture before deciding to treat.
