inv-01: Coverage criteria

Coverage is provided for diagnostic and monitoring scenarios as specified below, consistent with DHHS guidance.

ALL of the following

ONE of
- Baseline testing: Plasma HIV-1 or HIV-2 RNA quantification for baseline viral load when risk of infection is significant or when initiation of therapy is anticipated (examples: persistence of borderline/equivocal serology in an at-risk individual; signs or symptoms of acute retroviral syndrome).
- Monitoring disease progression: Plasma HIV-1 or HIV-2 RNA quantification for monitoring disease progression in known HIV-infected individuals per recommended intervals.
- Monitoring antiretroviral therapy (ART) response: Plasma HIV-1 or HIV-2 RNA quantification to assess response to ART with frequency per DHHS recommendations (entry, 2–4 weeks after initiation or modification, every 3–4 months during first 2 years, then every 6 months if stable).
- Infants born to HIV-positive mothers: Plasma HIV-1 or HIV-2 RNA quantification for infants younger than 18 months because maternal antibodies confound serology.
- Predicting maternal–fetal transmission: Plasma HIV-1 or HIV-2 RNA quantification to inform risk of maternal–fetal transmission and delivery planning (including third‑trimester assessment).
ALL of the following
- Assay consistency and limitations: Viral RNA levels should be measured using the same analytical method for a patient; a change in assay method necessitates re-establishment of a baseline.
- HIV-2 considerations: Quantitative HIV-2 testing is recommended before initiating ART and for monitoring, but HIV-2 quantitation is not widely available and some HIV-1 assays do not reliably detect HIV-2.

Guideline-derived scenarios for testing and monitoring

ALL of the following

Use combination Ag/Ab assays (HIV p24 antigen + antibody) for initial screening; reactive specimens should undergo antibody differentiation to distinguish HIV-1 from HIV-2.
If differentiation is negative or indeterminate after a reactive Ag/Ab screen, perform quantitative or qualitative HIV RNA; an undetectable RNA suggests the initial Ag/Ab was a false positive, whereas detectable RNA is consistent with acute infection and should prompt confirmation.

ALL of the following

A positive Ag/Ab or a positive HIV RNA result in the setting of a negative antibody differentiation test requires immediate confirmation on a new blood specimen before initiating ART.
In rare or inconclusive cases—for example, persons transitioning from PrEP—consider HIV DNA testing as adjunctive confirmation.

PrEP-specific considerationPersons on PrEP with low-positive RNA (<3,000 copies/mL) should have confirmatory antibody and quantitative RNA testing performed and available before initiating ART.

Likelihood of false positivesImprovements in viral load methodology make repeated false-positive HIV RNA test results unlikely; detectable RNA in this context is highly consistent with acute infection.

Prior Authorization

Confirmation before ART / special considerations for PrEP

Confirmation before initiating antiretroviral therapy (ART) or when special considerations for pre-exposure prophylaxis (PrEP) apply: A positive HIV Ag/Ab test result or a positive HIV RNA test result in the setting of a negative HIV antibody test result should prompt immediate confirmation of HIV diagnosis. Collect a new blood specimen to verify the HIV diagnosis before initiating ART. In persons taking PrEP who have a negative HIV antibody test and a very low-positive quantitative HIV RNA result (<3,000 copies/mL), repeat confirmatory HIV antibody testing and a repeat quantitative plasma HIV RNA test should be performed and results obtained before starting ART. In rare or inconclusive cases (for example when PrEP is being transitioned to a full ARV regimen and diagnostic testing remains unclear), consider HIV DNA testing as adjunctive confirmation.

Collect a new blood specimen for confirmation prior to ART initiation
