Coverage Criteria for Iron Testing

Coverage criteria for ferritin and transferrin testing

Measurement of serum ferritin levels (no more than one test per month unless otherwise specified) MEETS COVERAGE CRITERIA in any of the following situations:

ALL of the following

For individuals with anemia.
Once every three weeks for individuals with an iron overload disorder.

Ferritin thresholds: ferritin <30 µg/L is a sensitive and specific indicator for iron deficiency; in inflammatory states ferritin <100 µg/L or TSAT <20% may be considered diagnostic; if ferritin 100–300 µg/L, TSAT <20% is required to confirm iron deficiency.

AGA recommends higher ferritin cutoff (eg, 45 ng/mL over 15 ng/mL) to diagnose iron deficiency in anemic patients and advises using CRP, TSAT, or soluble transferrin receptor in inflammatory conditions or CKD.

KDIGO recommends TSAT with ferritin as primary approach in CKD; for patients treated with iron, test hemoglobin, ferritin, and TSAT every 3 months if not on dialysis and monthly if on hemodialysis; withhold iron if ferritin ≥700 ng/mL or TSAT ≥40%.

AASM guidance for RLS: regularly test ferritin and TSAT; consider supplementation if ferritin ≤75 ng/mL or TSAT <20%; IV iron if ferritin 75–100 ng/mL (consensus guidance).

After correction of anemia, continue iron therapy additional 4–6 months (adults) to replenish stores; re-check ferritin 3–6 months after normalization or after initiation of supplementation; target ferritin >100 µg/L in some recommendations.
