EmblemHealth will reimburse discarded amounts from single‑use vials/packages when the following criteria and documentation requirements are met. Multi‑dose vials/multi‑use packages are excluded.

- Providers must bill using the smallest available manufacturer vial size (or combination of vials) that could provide the appropriate dose to minimize waste; excess discarded amounts greater than the minimum possible will be denied as bundled/included in administered drug reimbursement.

- Medical record must include physician orders for the drug; amount administered; total amount the vial is labeled to contain; date/time the drug was discarded; amount discarded; and name, licensure, and signature of the person who wasted the drug.
- Discarded amount must be documented in the same location as administration; charge capture reports are not acceptable documentation.

- EmblemHealth may request medical records (internal or external/vendor reviews); records received in response to requests are considered the complete documentation for review and additional records received later will not be accepted.
- If records do not adequately document physician order, administered amount, and discarded amount, excess billed amounts will be denied; providers may appeal denials but may not submit additional records that were not part of the original review.

Exclusions — do not bill for any of the following

Discarded/wasted amounts from multi‑dose vials or multi‑use packages are not reimbursed.
Discarded drugs if none of the drug was administered to the patient.
Claims with JW where a corresponding administered drug line does not exist for the same drug on the same date of service.
Claims reporting identical drugs on the same date of service with both JW and JZ appended.
Claims for drugs with no discarded amount that do not have modifier JZ on the administered drug line for the same date of service.
Drugs billed without using the most appropriate vial size or combination of vials to deliver the administered dose.

Effective 10/1/2024, EmblemHealth requires the JZ modifier on all claim lines that bill drugs from single‑dose containers when no discarded amount exists; the administered drug line must include the drug billing code, the JZ modifier, and the number of units administered. To bill discarded amounts, append modifier JW on a separate claim line with a valid NDC and follow the smallest‑vial and units‑billing rules; do not use JW for multi‑dose vials. EmblemHealth may request medical records and will deny waste claims that are insufficiently documented or that bill excess discarded amounts.

For administered drug with no waste: include the HCPCS/CPT code, JZ modifier, and units administered on the claim line (required effective 10/1/2024).
To bill discarded waste: report administered amount on one line and report discarded amount on a separate line with modifier JW plus a valid NDC; bill units in multiples of the CPT/HCPCS descriptor and use smallest available vial(s).
Do not append JW if no discarded amount is billed, and do not use JW for multi‑dose vials or when actual dose administered is less than the billing unit.
