Rhinoplasty Coverage Criteria

Covered Indications

Members are eligible for rhinoplasty when either of the following criteria is met:

Path 1: Concomitant with septoplasty
- Performed in conjunction with a septoplasty for nasal airway obstruction
  The nasal deformity must contribute to the airway obstruction and septoplasty criteria must be met.
Path 2: Reconstructive rhinoplasty for severe deformity
- Documented obstructive breathing symptoms secondary to one or more of the following:
  - Excision of a nasal abscess
  - Excision of a malignant mass
  - Osteomyelitis
  - Cleft lip and/or palate repair
  - Nasal trauma or injury that resulted in significant deviation of the nasal pyramid or creation of a significant dorsal hump
  - Nasal dermoid
  - Saddle nose deformity from surgery, trauma, or disease (e.g., Granulomatosis with Polyangiitis)
Path 3: Vestibular stenosis
- Vestibular stenosis causing prolonged nasal obstruction that is moderate to severe
- Obstruction must be separate from obstruction caused by a deviated septum or turbinate hypertrophy

Rhinoplasty is not covered when it is performed solely to change appearance in the absence of any signs or symptoms of functional abnormalities or nasal defects. Such procedures are considered cosmetic and are excluded from coverage.

Procedures performed solely for cosmetic appearance without evidence of functional nasal abnormality are considered not medically necessary and will not be covered under this guideline.

EmblemHealth cannot properly review or approve a prior authorization request without the required documentation.

Background

Rhinoplasty is a surgical procedure of the nose intended to correct external nasal deformity while maintaining or improving nasal function. EmblemHealth considers rhinoplasty medically necessary when used to address functional airway obstruction in conjunction with septoplasty, for reconstruction following significant deformity or trauma, or for treatment of prolonged vestibular stenosis that causes moderate to severe obstruction. Procedures performed solely for cosmetic reasons, without functional abnormalities, are excluded from coverage; additionally, certain devices (for example, absorbable nasal implants such as Latera, CPT 30468) are considered investigational for Commercial and Medicaid.
