Coverage Criteria for Home Uterine Activity Monitoring (HUAM)

COVERAGE CRITERIA

Covered when any ONE of the following is met for members receiving high-risk prenatal care evaluated by a perinatologist:

HUAM eligibility

History or current PTL with documented cervical changes: History or current preterm labor with documented cervical changes (dilation or effacement).
Multiple gestation pregnancy (≥ 2).
Contractions not self-detectable secondary to physiologic condition (eg, neuromuscular disease, paralysis).
Positive fetal fibronectin with progressive cervical changes: Positive fetal fibronectin test, gestational age ≥20 weeks and <36 weeks, and progressive cervical changes with cervical length < 2.5 cm documented by vaginal probe ultrasound despite treatment with multiple tocolytics.
Risk factors increasing preterm birth with inability to tolerate contractions: Presence of risk factors that increase risk of preterm birth (eg, placenta previa with hemorrhage; history of classical cesarean section or deep myomectomy) with contractions or cervical change in the index pregnancy such that the woman is not able to safely tolerate contractions.
Incompetent cervix as documented by need for cerclage or signs of cervical incompetence (eg, funneling on Valsalva, silent shortening).
