Medically Necessary Coverage Criteria

Members receiving high risk prenatal care who have been evaluated by a perinatologist are eligible for HUAM coverage when ANY of the following criteria are met:

History or current preterm labor (PTL): History or current PTL with documented cervical changes (dilation or effacement)
Multiple gestation pregnancy (≥ 2)>= 2
Covered for twins during COVID emergency (historical revision noted)
Non-detectable contractions: Contractions not self-detectable secondary to physiologic condition (ie, neuromuscular disorder, paralysis, etc)
Positive fetal fibronectin with progressive cervical change: Women with a positive fetal fibronectin test and progressive cervical changes despite treatment with multiple tocolytics>= 20 weeks and < 36 weeks; cervical length < 2.5 cm (documented by vaginal probe ultrasound)
Document gestational age, positive fetal fibronectin, cervical length <2.5 cm by vaginal probe ultrasound, and failure of multiple tocolytics.
Other high-risk factors: Presence of risk factors that increase risk of preterm birth (eg, placenta previa with hemorrhage; history of classical cesarean section or deep myomectomy with contractions/cervical change in index pregnancy such that woman cannot safely tolerate contractions)
Incompetent cervix as documented by need for cerclage or signs of cervical incompetence (funneling on Valsalva, silent shortening)
