Coverage Criteria for Intravenous Belimumab (Benlysta)

Initial Therapy - Systemic Lupus Erythematosus (SLE)

Covered when ALL of the following are met

SLE initial criteria: A. Patient is 5 years of age or older;

SLE initial criteria: B. Patient must NOT have an active infection;

SLE initial criteria: C. Patient has NOT received a live vaccine within 30 days before starting or concurrently with Benlysta;

SLE initial criteria - exclusions: D. Patient does NOT have any of the following exclusion criteria:

Exclusion details: i. Severe active central nervous system lupus;

Exclusion details: ii. Individuals who are on rituximab, anifrolumab, or IV cyclophosphamide;

SLE initial criteria: E. Patient has a confirmed diagnosis of SLE with at least 4 diagnostic features (one of which must include a positive autoantibody test such as ANA and/or anti-dsDNA >2x lab reference if tested by ELISA);

SLE initial criteria: F. Patient has failed to respond adequately to at least two (2) standard therapies (examples: anti-malarials, corticosteroids, NSAIDs, immunosuppressives — excluding IV cyclophosphamide);

G. Disease activity (one required)

Exclusions: Benlysta is not appropriate for patients with severe active central nervous system lupus. Concurrent use of certain biologics or potent immunomodulatory agents is prohibited: for systemic lupus erythematosus (SLE) the policy excludes individuals who are on rituximab, anifrolumab, or IV cyclophosphamide; for lupus nephritis the policy excludes concurrent use of rituximab or anifrolumab (these agents must not be co-administered with belimumab while meeting initial-therapy criteria).

Concurrent therapy rules: before authorizing IV belimumab, verify the member is not currently receiving any excluded agents listed above. If the member has recently received one of these agents, the treating clinician should document timing and rationale; concurrent administration with the excluded biologics/IV agents is not supported by this policy.

Requests that do not meet the specified initial-therapy criteria are not supported. Examples include absence of required prior-therapy failure (for SLE the member must have failed at least two standard therapies; for lupus nephritis the member must have failed corticosteroids plus either cyclophosphamide or mycophenolate mofetil), insufficient diagnostic confirmation (fewer than 4 SLE diagnostic features or lack of a biopsy-proven Class III, IV, or V lupus nephritis), active infection, or recent live vaccination within 30 days. Such gaps in the documented criteria will render the use not medically necessary under this policy.

Providers should submit clear documentation of prior therapies tried and inadequate response, and the required diagnostic evidence (including renal biopsy results and baseline renal measures for lupus nephritis) to support medical necessity. Absent this documentation, prior authorization requests may be denied per the policy.

Background

Benlysta (belimumab) is a human IgG1 lambda monoclonal antibody that targets soluble B-lymphocyte stimulator protein (BLyS), inhibiting its binding to B-cell receptors. By neutralizing BLyS activity, belimumab reduces B-cell survival and activity, providing a targeted approach for treating systemic lupus erythematosus and, when criteria are met, lupus nephritis.
