Coverage criteria for Vectibix (panitumumab)

Colorectal cancer indications

Covered when ALL of the following are met for colorectal cancer:

Colorectal cancer, anti-EGFR standard use: Member has advanced or metastatic colorectal cancer with KRAS/NRAS/BRAF wild-type gene status

Required for standard anti-EGFR combinations

Permitted clinical settings: Vectibix used in combination with FOLFOX or FOLFIRI, or as single agent or with irinotecan for subsequent therapy following prior chemotherapy

Capecitabine (Xeloda) may substitute for 5-FU in 5-FU–based regimens

BRAF V600E initial-line option: For stage IV metastatic BRAF V600E mutation positive colorectal cancer, Vectibix may be used in combination with encorafenib (Braftovi) and FOLFOX; KRAS/NRAS may be wild-type or unknown

Member must have no prior selective BRAF inhibitor or EGFR inhibitor treatment; tumors should not be MSI‑H/dMMR unless ineligible for or progressed on immune checkpoint inhibitors

BRAF V600E subsequent-line option: Vectibix may be used as subsequent therapy with encorafenib for unresectable/metastatic BRAF V600E mutation positive disease regardless of KRAS/NRAS status

Use of Vectibix (panitumumab) is excluded for members who have documented disease progression while receiving panitumumab or who have previously failed therapy with cetuximab (Erbitux).

Requests lacking documented tumor genotyping (KRAS/NRAS/BRAF) and corresponding test results for indications that require these biomarkers will be denied. For standard anti‑EGFR use in colorectal cancer, documentation of appropriate genotype testing is required.

Dosing that exceeds a single dose of 6 mg/kg is not covered. Additionally, combinations or indications considered investigational and not supported by CMS‑recognized compendia or acceptable peer‑reviewed literature are excluded.

Off‑label uses of Vectibix (panitumumab) that are not supported by CMS‑recognized compendia or acceptable peer‑reviewed literature are considered not medically necessary. Evidence must adequately represent the patient and tumor characteristics, the regimen under study, and report clinically meaningful outcomes (for example ASCO‑recommended thresholds such as hazard ratios < 0.80 and survival benefits of at least 3 months for OS or PFS).

Case reports, meeting abstracts without full peer‑reviewed publication, or other anecdotal evidence alone are generally insufficient to establish medical necessity for off‑label indications.

Continuation requests may be exempt when the member has not experienced disease progression on the requested medication, the medication was used within the last year without a gap of more than 30 days of active authorization, and no additional medications are being added to the continuation request.

Vectibix must be used in appropriate sequence relative to other therapies and meet prior treatment considerations: for colorectal cancer, Vectibix is indicated for KRAS/NRAS/BRAF wild-type advanced or metastatic disease and may be used in combination with FOLFOX or FOLFIRI, or as a single agent or with irinotecan for subsequent therapy after prior chemotherapy for metastatic disease. Capecitabine (Xeloda) may substitute for 5-FU in 5-FU–based regimens.
