This document announces that Culinary Health Fund will waive copayments and coinsurance for participants and dependents injured in the October 1 Las Vegas Strip tragedy, including medical and mental health outpatient therapy visits for a specified period.

Cost-sharing waiver criteria
Waiver applies when ALL of the following are met:
ALL of the following
- Member or dependent was injured as a result of the October 1 Las Vegas Strip tragedy.
- The service is a covered service related to the injury (examples include but are not limited to hospitalizations, office visits, physical therapy).
- Member copayments and coinsurance for those covered services are waived.
ALL of the following
- Mental health outpatient therapy visits related to the October 1 tragedy.
