Coverage and Medical Necessity Criteria

Initial diagnostic/therapeutic SIJ injection

Diagnostic or therapeutic SIJ injections are medically necessary when ALL of the following are met:

Core eligibility: Member has moderate to severe low back pain primarily over the anatomical SI joints for at least 3 months; pain located below L5 without radiculopathy; at least three positive SIJ provocation tests; failure to respond to a minimum of four weeks of conservative therapy; clinical findings and/or imaging do not suggest another diagnosed or obvious cause of the lumbosacral pain; one injection per visit.

Procedural requirements for injections: SIJ injections (diagnostic or therapeutic) must be performed under CT or fluoroscopy image guidance with contrast (ultrasound only reasonable for documented contrast allergy or pregnancy); injections must not be performed with other lumbosacral musculoskeletal injections; documentation must show direct causal benefit from the SIJ injection and not from other musculoskeletal injections or treatments.

Second diagnostic/confirmatory SIJ injection is medically necessary when ALL of the following are met:

Confirmatory block criteria: Pain improved by at least 75% after the first diagnostic SIJ injection; at least two weeks have passed since the initial injection.≥75% and ≥2 weeks

Subsequent therapeutic injections

Subsequent therapeutic SIJ injections for recurrence of pain are medically necessary when ALL of the following are met:

Efficacy: Prior therapeutic SIJ injection produced at least 50% consistent pain relief or at least 50% consistent improvement in ability to perform previously painful movements/ADLs for at least 3 months from the proximate therapeutic SIJ injection compared to baseline using the same pain scale.≥50% / ≥3 months

Adjunctive care and interval: Request is for SIJ injection administered for temporary relief in conjunction with other noninvasive treatments (not as stand-alone therapy); injections are given at intervals of at least 2 months apart.≥2 months between injections

Frequency limit per site: Subsequent therapeutic SIJ injections are provided at the same anatomic site as the initial therapeutic SIJ injection and fewer than four therapeutic SIJ injections have been given at the same site in the last 12 months.

Not medically necessary interventions

Non-coverage - sacroiliac nerve blocks: Sacroiliac nerve blocks are considered not medically necessary because effectiveness has not been established.

Non-coverage - radiofrequency neurotomy: Radiofrequency neurotomy (conventional, cooled, and pulsed) of the sacroiliac joint is considered not medically necessary because effectiveness has not been established; high-quality studies are lacking and additional well-designed studies are needed for cooled techniques.

Continuation beyond 12 months

Continuation beyond 12 months may be considered on a case-by-case basis when ALL of the following are met:

Continuation criteria: Pain causes significant functional or vocational disability with measurable goals and objective scales; SIJ injections have provided at least 50% sustained and consistent improvement in pain and/or function for at least 3 months; documentation includes rationale such as high surgical risk, patient declines surgery, or recurrence sustained and consistently relieved by SIJ injections.

Primary care provider should be notified regarding continuation and prolonged steroid use.

SIJ injections should not be performed when clinical findings or imaging studies indicate an alternative cause of lumbosacral pain. Examples include: central spinal stenosis with neurogenic claudication or myelopathy, foraminal stenosis or disc herniation with concordant radiculopathy, infection, tumor, fracture, pseudoarthrosis, or pain related to spinal instrumentation. Documentation must exclude these alternate diagnoses before pursuing SIJ injection.

Sacroiliac nerve blocks are considered not medically necessary because effectiveness has not been established. Radiofrequency neurotomy of the sacroiliac joint — including conventional, cooled, and pulsed techniques — is also considered not medically necessary due to insufficient high-quality evidence supporting effectiveness; additional well‑designed studies are needed.

This clinical policy provides guidance on medical necessity for sacroiliac joint interventions to assist coverage decisions and benefit administration. It is not a substitute for clinical judgment and does not dictate the care a provider should deliver. Providers are expected to exercise professional medical judgment in diagnosing and treating members, and coverage remains subject to the member's specific benefit terms, state and federal requirements, and applicable Health Plan administrative policies.

Conservative Care Requirements Before Intervention

Document failure of conservative care — minimum duration

Documented failure of conservative care is required prior to SIJ injections:

Conservative care requirement: Failure to respond to a minimum of four weeks of conservative therapy, defined as an appropriate combination of therapeutic medications in therapeutic dosages plus nonpharmacologic measures (e.g., physical therapy, spinal manipulation, cognitive behavioral therapy, home exercise program) tailored to the patient.minimum 4 weeks

Denial of requested SIJ injections may occur when documentation is insufficient, the clinical criteria are not met, or benefit terms/limitations apply.

Denial triggers include: failure to meet listed medical necessity criteria (e.g., duration/location of pain, positive provocation tests, failure of conservative therapy, imaging excluding other causes).

Background and Clinical Context

Sacroiliac joint (SIJ) pain is an identifiable contributor to chronic low back pain in a substantial minority of patients. First-line management is conservative care — for at least a minimum of 4 weeks — including appropriately dosed medications plus nonpharmacologic measures such as physical therapy, spinal manipulation, cognitive behavioral therapy, and a home exercise program. When conservative measures fail and clinical assessment (including at least three positive SIJ provocation tests) and imaging do not indicate an alternate lumbar source, image‑guided intra-articular SIJ injections (performed with CT or fluoroscopy and contrast, with ultrasound only for documented contrast allergy or pregnancy) may be used for diagnostic confirmation and/or temporary therapeutic relief.
