Coverage Criteria for Home Ventilators

Initial therapy — restrictive thoracic/neuromuscular disorders

Covered when ALL of the following are met for initial request for first three months:

Initial criteria for restrictive thoracic or neuromuscular disease: 1. Documentation of a neuromuscular disease (e.g., amyotrophic lateral sclerosis) or a severe thoracic cage abnormality (e.g., post‑thoracoplasty for tuberculosis or severe kyphoscoliosis) and one of the following measured while the member/enrollee was stable (not in acute respiratory failure): (a) arterial blood gas PaCO2 ≥ 45 mm Hg while awake and breathing room air or prescribed oxygen; OR (b) sleep oximetry demonstrating O2 saturation ≤ 88% for at least five minutes while breathing prescribed oxygen. If neuromuscular disease is present, maximal inspiratory pressure (MIP) < 60 cm H2O or forced vital capacity (FVC) < 50% predicted may apply. 2. Documentation that the member/enrollee could not be appropriately treated with a respiratory assist device (RAD). 3. Documentation that the noninvasive home ventilator will not be used to provide RAD or CPAP therapy (i.e., will be used to provide average volume assured pressure support). 4. Chronic obstructive pulmonary disease (COPD) does not contribute significantly to the pulmonary limitation.see nodes

Initial criteria for COPD: 1. An arterial blood gas measurement showing PaCO2 ≥ 52 mm Hg measured while awake at baseline on prescribed FiO2. 2. Prior to initiating therapy, sleep apnea and treatment with CPAP has been considered and ruled out (formal sleep testing is not required if the medical record documents that sleep apnea is not the predominant cause of awake hypercapnia or nocturnal arterial oxygen desaturation). 3. Documentation that the member/enrollee could not be appropriately treated with a respiratory assist device (RAD). 4. Documentation that the noninvasive home ventilator will not be used to provide RAD or CPAP therapy (i.e., will be used to provide average volume assured pressure support).see nodes

Invasive ventilator coverage: Initial and ongoing use of an invasive ventilator is medically necessary for long‑term or chronic conditions that impair the ability to maintain adequate respiratory status (examples include neuromuscular disease, thoracic restrictive disease, or chronic respiratory failure following COPD).

Second or backup ventilator

Second or backup ventilator: A second ventilator is medically necessary when it serves a different purpose from the first (e.g., negative‑pressure ventilator with chest shell for part of the day and a positive‑pressure ventilator with mask for other periods), or as a back‑up when needed (for example, a wheelchair‑mounted ventilator for daytime mobility plus a ventilator for bed use, or for residence in remote areas with poor emergency access).

Requests for a noninvasive home ventilator (NHV) must demonstrate that the device is intended to provide ventilatory support beyond what a respiratory assist device (RAD) or simple CPAP can deliver. Specifically, initial coverage requires documentation that the patient could not be appropriately treated with a RAD and that the NHV "will not be used to provide RAD or CPAP therapy (i.e., will be used to provide average volume assured pressure support)." Do not request an NHV if the intended primary use is RAD/CPAP support. (See COPD and restrictive/neuromuscular criteria for physiologic thresholds.)

Devices that function primarily as RAD or CPAP are excluded from coverage under the NHV criteria when RAD/CPAP would be appropriate and sufficient. The policy explicitly states that the NHV must not be used to provide RAD or CPAP therapy and that, for coverage, COPD must not contribute significantly to the pulmonary limitation unless the COPD-specific PaCO2 threshold is met. If COPD substantially contributes to respiratory impairment but the COPD PaCO2 threshold is not met, an NHV is not supported.

Prior authorization is required for initial 3-month trial requests and for continued coverage of noninvasive home ventilators. Providers must submit a prior authorization request before device provision to avoid claim denials or delays.

Step Therapy

Attempt Less Intensive Therapies First

Providers must document that less intensive therapies (RAD or CPAP) were attempted or are inadequate/unsuitable prior to authorization of a noninvasive home ventilator. If CPAP or RAD could appropriately treat the member, NIV will be denied.

For restrictive thoracic disorders and neuromuscular disease: documentation that the member "could not be appropriately treated with a respiratory assist device (RAD)" and that the NHV "will not be used to provide RAD or CPAP therapy."

Background

Noninvasive ventilation (NIV) delivers positive pressure via external interfaces (masks, mouthpieces, nasal pillows) to support ventilation without an invasive airway, while invasive ventilation uses an invasive interface such as a tracheostomy or endotracheal tube. Home NIV (including noninvasive home ventilators that provide average volume assured pressure support) is preferred when feasible because it can improve quality of life and reduce acute care needs. An NHV differs from standard BiPAP/RAD by delivering variable airway pressures to reach a targeted tidal volume when simpler devices cannot meet ventilatory requirements.

Exclusions and Not Covered Items

A noninvasive home ventilator is not covered when a less intensive therapy would be appropriate and sufficient. Specifically, the policy lists provision of an NHV as not covered when the patient could be adequately managed with a respiratory assist device (RAD), CPAP, or BiPAP at optimal settings, or when the required physiologic thresholds (for example, the COPD PaCO2 threshold) are not met. Providers must therefore document attempts and/or inadequacy of those less intensive modalities before an NHV can be authorized.
