Coverage criteria for pediatric/neonatal critical and intensive care

Reimbursement follows AMA CPT instructions and CMS NCCI edits for same-day reporting by providers in the same group.

When pediatric or neonatal critical and intensive care codes (99468-99476, 99477-99480) are reported on the same day with other specified procedure services for the same patient by physicians and/or Other Qualified Health Care Professionals in the same group, apply CPT reporting instructions to determine which services are considered inclusive and which codes to report.

If services considered inclusive per CPT are reported on the same day with a pediatric or neonatal critical/intensive care code by the Same Group Physician and/or Other Qualified Health Care Professional, those services will be evaluated for separate reimbursement or bundled into the critical/intensive care code in accordance with CMS NCCI edits.

When a modifier override is being considered to report a specified procedure separately from a pediatric or neonatal critical/intensive care code on the same day by the Same Group Physician and/or Other Qualified Health Care Professional, UnitedHealthcare will follow CMS NCCI guidance to determine if modifier override is appropriate. Modifiers referenced include: 59, XE, XS, XU.

Report and bundle pediatric/neonatal critical & intensive care per CPT and NCCI

Follow AMA CPT instructions for reporting pediatric and neonatal critical and intensive care codes (99468-99476; 99477-99480). When those critical/intensive care services are reported on the same day with other specified procedure services for the same patient by providers of the same group, UnitedHealthcare will apply NCCI edits to determine whether the other services are considered inclusive (bundled) or eligible for separate reimbursement. For same-day E/M interactions, refer to UnitedHealthcare Community Plan's Same Day/Same Service and Rebundling policies for additional determinations.
