Observation service coverage criteria

Covered when ALL of the following criteria are met:

Observation services (G0378) must be billed with type of bill 13X, 78X, or 85X and reported under revenue code 0762; units must represent hours of observation rounded to the nearest hour.

The reported observation period for G0378 must meet or exceed 8 hours to be considered for reimbursement.

Observation services must be reported on a single claim line using the date of service on which observation care begins; do not span dates or split into multiple claim lines even if observation crosses calendar days.

ALL of the following

Emergency department visit (CPT 99281-99285 or G0380-G0384)
Hospital outpatient clinic visit (HCPCS G0463)
Critical care (CPT 99291)
Direct referral/admission to observation care reported with HCPCS G0379 (G0379 must be reported on the same date of service as G0378)

G0379 (direct admission to observation) should be reported only when the patient was referred directly from the community and there was no ED visit, outpatient clinic visit, or critical care service on the day observation begins; G0379 must be submitted on the same date as G0378 and is not separately payable if an ED visit, clinic visit, critical care, or a service with IOCE status indicator T or V is reported the same date.

G0378 will not be reimbursed when reported in addition to procedure codes assigned status indicator J1 or T under the CMS IOCE, or when observation monitoring is inclusive of payment for a surgical, diagnostic, or therapeutic procedure (e.g., monitoring before a decision to proceed to surgery, during surgical recovery, or routine prep/recovery for a diagnostic test).

Timekeeping/rounding: round start and end times to the nearest hour to calculate units (example: 10:15 -> 10:00; 6:52 -> 7:00).

Example: observation from 10:00 PM Friday to 4:00 PM Saturday is reported on one line with 18 units and Friday as the date of service.

G0378 should be reported only when an ED visit (99281-99285, G0380-G0384), clinic visit (G0463), critical care (99291), or direct referral/admission to observation (G0379) was provided on the same date or the day before. Do not report observation care that is inclusive of or included in payment for a surgical, diagnostic, or therapeutic procedure; G0378 will not be reimbursed when reported in addition to procedure codes assigned status indicator J1 or T under the CMS IOCE.
