Evidence base and review: The guidelines cite ACR Appropriateness Criteria, peer-reviewed studies, and other authoritative sources as the primary evidence base. The guideline set is formally reviewed and updated regularly (annual formal review with interim updates as needed) as documented in the policy history and prefatory sections.

Definitions

Concurrent supervision: Concurrent supervision is defined as active physician participation and monitoring of the 3D reconstruction process, including anatomic design, determination of tissue types/structures to display, selection of images/cine loops to archive, and monitoring/adjustment of the 3D work product.

Unit of service for guidance codes: The unit of service for guidance codes is the individual encounter (date of service), not the number of lesions, aspirations, biopsies, injections, or localizations. Only one unit of any radiologic guidance code should be reported per encounter.

Unit of service (guidance codes): For CT/MR/US guidance and related codes (e.g., 77012, 77021, 76942, 77002-77003, 77013, 77022), billing must reflect a single unit per encounter and these guidance codes include the imaging necessary to guide percutaneous procedures.

WBMRI reporting: Whole-body MRI has no established CPT/HCPCS codes and should be reported as CPT 76498 when appropriate (WBMRI generally not supported except select indications).

Provider-directed conservative treatment: Provider-directed conservative treatment means non-surgical measures directed by the treating provider (e.g., rest, immobilization, physical therapy, medications) with clinical re-evaluation; duration requirements are specified per indication and commonly require 6 weeks when indicated.

Advanced imaging definition: Advanced imaging refers to US, MRI, CT, and Quantitative CT (QCT) as referenced by CPT codes in the guideline and is distinguished from initial/conventional imaging (plain radiographs).

CT vs MRI substitution and contrast considerations

Apply ALL of the following decision rules when choosing CT versus MRI and when contrast is considered:

Appropriateness for substituting CT for MRI (or vice versa):

• Use MRI preferentially for soft-tissue, marrow, occult stress/insufficiency fractures, inflammatory arthritis, osteonecrosis, meniscal and ligamentous injuries when available and not contraindicated.

• Use CT preferentially for fine cortical detail, complex fracture characterization, preoperative bony surgical planning, and when MRI is contraindicated or unavailable.

Contraindications and considerations:

• MRI contraindications (e.g., pacemaker/ICD not MRI-conditional, certain implanted devices, severe claustrophobia, inability to cooperate) justify CT when CT can answer the clinical question.

• CT iodinated contrast is indicated when vascular detail or enhancement characterization is necessary; contraindications to iodinated contrast (severe allergy not amenable to premedication, renal insufficiency) should be documented and alternate modalities considered.

Operator: all of the following must be met when making substitution/contrast decisions:

• Documentation of clinical reason for preferring one modality over another in the request or chart.

• Prior plain radiographs unless an exception applies.

• If contrast is to be administered unexpectedly based on findings during a non-contrast study, documentation in the imaging record is required.

CT/MR/US Guidance Codes and Billing Rules

Billing and coding guidance for combined-modality and guidance-code use. Apply ALL rules below:

Guidance codes (e.g., 77012, 77021 where applicable) should be billed only when the documented service meets the CPT descriptor and when a separate payable component exists beyond the primary imaging study.

When a guidance code is reported in conjunction with an advanced imaging study, documentation must show distinct additional work or equipment use (e.g., separate real-time imaging guidance for percutaneous access) to support separate billing.

Do not unbundle guidance codes from the primary imaging study when the guidance is inherent and integral to the billed advanced imaging procedure based on CPT definitions.

Modifiers and site-specific billing rules must follow payer-specific rules; when guidance codes are reported with therapeutic/procedural CPTs, ensure modifier usage (e.g., -59/XS) is supported and documented.

All billed codes must reflect the actual work performed and be supported in the imaging/procedural report to avoid denials for unbundling or lack of medical necessity.

Specific CPT guidance (selected examples)

Selected CPT-specific billing examples and rules (apply ALL that follow):

Breast biopsy guidance: CPT 19085/19086 — select the code that matches the imaging modality and approach; document localization method, laterality, and whether specimen radiograph was performed.

Stereotactic/percutaneous procedures with imaging: use the CPT descriptor to select the correct guidance code; document separate guidance time and complexity if reporting as distinct from the primary procedure.

Fluoroscopic or CT guidance for injections/biopsies: when both guidance and procedural codes are billed, document distinct elements supporting each code (planning, guidance imaging, needle manipulation, confirmation of placement).

When guidance codes pair with code pairs such as 77013/20982 or other device-implantation codes, follow CPT pairing rules and payer modifier guidance; ensure documentation supports separate payable components.

General Criteria for Advanced Musculoskeletal Imaging

General prerequisites and conservative care requirements for advanced musculoskeletal imaging. Apply ALL of the following unless condition-specific exceptions exist:

Plain radiographs from the current episode of symptoms or since the change in symptoms are required before authorization of most advanced imaging studies.

Conservative therapy and/or a period of appropriate non-invasive management is required for many non-acute musculoskeletal complaints prior to advanced imaging. Conservative care may include activity modification, analgesics/NSAIDs, brief physical therapy trial, immobilization, and topical measures as clinically appropriate.

Documented failure or inadequate response to conservative measures must be included in the clinical record and authorization request when conservative care is required by the condition-specific guideline.

Exceptions: acute trauma with concern for complex fracture, neurologic deficit, suspected septic joint/osteomyelitis, or surgical planning where conservative care would be inappropriate or harmful.

Nested conservative care detail (ALL):

• Conservative care must be contemporaneous with the current episode of symptoms; list types, start dates, duration, and response.

• If conservative care was contraindicated or not feasible, documentation must explain the reason and the clinical judgment that led to immediate advanced imaging.

3D Rendering (CPT 76376 / 76377) - Appropriateness

3D rendering (CPT 76376/76377) is appropriate when ALL of the following are satisfied:

Clinical scenarios where 3D rendering may be appropriate (ANY one of):

• Complex fracture characterization that will change surgical approach or fixation strategy.

• Preoperative planning for complex joint reconstruction, osteotomy, or deformity correction where 3D models alter surgical planning.

• Mandible/maxillofacial, pelvis, acetabular, or other anatomically complex surgical planning requiring spatial relationship visualization beyond standard multiplanar reconstructions.

Operational constraints (ALL must be met):

• 3D rendering is performed on a workstation distinct from the primary modality console and after the primary interpretation is completed.

• Documentation must state the clinical question addressed by the 3D rendering and how the rendering changed management or surgical planning.

• CPT 76376 is for simple 3D reconstruction; CPT 76377 is for complex/interactive rendering per CPT definitions — select the correct code per the work performed and document accordingly.

• Supervision and reporting: the rendering physician must document the methods and findings in the medical record; technologist-only post-processing without physician interpretation does not support physician-level CPT reporting.

Unlisted CT/MR/Nuclear Procedure Codes (76497/76498/78999)

Report unlisted CT/MR/nuclear procedure codes (e.g., 76497, 76498, 78999) only when ALL of the following apply:

The procedure performed does not have a specific CPT code that accurately describes the service.

A detailed report and supporting documentation (including indication, technique, images, and rationale why no listed CPT applies) are submitted with the claim to allow review and appropriate valuation.

If an established CPT code exists that reasonably describes the service, that code should be used instead of an unlisted code.

When using unlisted codes for novel or uncommon procedures, include operative/procedural notes, duration, complexity, and any work relative value justification to support medical necessity and reimbursement.

Limited CT (CPT 76380)

Limited CT (CPT 76380) is appropriate when ALL of the following are true:

Use limited CT for focused, non-contrast localizer or scout-type acquisitions that answer a narrowly-defined clinical question (examples: small field evaluation for radiolucent foreign body localization in the extremity, focused lucency near an implant).

Limited CT should NOT be used for radiation therapy treatment planning, comprehensive anatomic surveys, or when full diagnostic CT with multiplanar reconstructions is required.

Document the focused clinical question and the limited anatomical coverage on the imaging request and report.

SPECT/CT and Nuclear Procedure Coding

Hybrid SPECT/CT and nuclear procedure coding rules (ALL):

Report the nuclear medicine portion with the applicable nuclear CPT code(s) and report the CT portion separately when the CT meets the definition of a diagnostic CT (i.e., separate technical and professional components).

When CT is limited to attenuation correction or localization only and per CPT descriptors is considered integral, do not separately bill a diagnostic CT code unless additional diagnostic CT work was performed and documented.

Selecting CPT codes: choose codes that match the diagnostic extent and intent of each modality component; provide documentation distinguishing attenuation correction/localization CT from a diagnostic CT.

Secondary Interpretation

Interpretation of outside examinations and secondary reads follow ALL of the rules below:

When interpreting external imaging, the interpreting physician must state in the report that images from an outside facility were reviewed, list the studies reviewed (modality, date, and facility), and describe any limitations of the outside study.

CPT 76140 may be reported when a separate, medically necessary, second interpretation of an outside diagnostic imaging study is performed and documented; ensure payer-specific requirements for reporting 76140 are met.

Do not report secondary interpretation codes when reinterpretation is routine or bundled within the primary professional read per contract or payer policy.

Quantitative MR Analysis

Quantitative MR analysis codes (Category III mpMRI and other quantitative MR codes) are covered only when ALL apply:

The quantitative MR procedure is supported by condition-specific guideline criteria that reference these Category III codes or other validated, disease-specific quantitative protocols.

The request includes documentation of the specific quantitative technique, clinical rationale, and how the results will change management.

If the quantitative analysis is investigational or lacks clinical utility for the indication, it is not covered. See condition-specific sections for references to quantitative MR use.

HCPCS Code Handling

HCPCS codes reported in musculoskeletal imaging should be handled as follows (ALL):

Treat HCPCS codes using the same clinical and coding rules as CPT codes; ensure the HCPCS descriptor matches the service performed.

If a HCPCS code is non-specific or ambiguous for the musculoskeletal indication, map the service to the appropriate CPT code when possible or provide detailed documentation supporting the HCPCS selection.
