Observation services are covered when consistent with InterQual criteria and the independent clinical judgment of a qualified provider. Observation is commonly appropriate for the following conditions (not all-inclusive):

Observation should be reserved for appropriate clinical indications; prolonged observation beyond expected improvement (typically >48 hours) without supporting documentation may result in denial.
